Most Americans have never heard of the SNAP hot food ban. Those who have tend to encounter it the hard way, at a register, with a cart full of groceries and a chicken they cannot buy. But to understand why the rule exists, and why it has proven so stubbornly difficult to change, you have to go back nearly a century, to a government more interested in saving farms than feeding people.

It Started With Surplus, Not Hunger

The roots of American food assistance stretch to 1939 and the depths of the Great Depression, when the federal government faced a peculiar crisis: farmers were destroying surplus crops and livestock they could not sell, while millions of Americans stood in breadlines unable to afford groceries. The government’s solution was to distribute surplus food to the poor, helping struggling Americans while, more importantly to the politicians involved, stabilizing farm prices and protecting agricultural interests. From the very beginning, feeding the hungry was tangled up with protecting agribusiness.

The original program let people on relief buy orange stamps equal to their normal food expenditures, receiving 50 cents worth of blue stamps for every dollar spent. Orange stamps could buy any food. Blue stamps could only be used for government-designated surplus items. It was less a nutrition program than a market-stabilization scheme wearing a humanitarian hat.

The 1964 Compromise That Shaped Everything

The modern program took shape in 1964, when President Lyndon Johnson signed the Food Stamp Act as part of his War on Poverty. Getting it passed required classic political horse-trading: urban Democrats agreed to vote for wheat and cotton legislation only if rural Democrats agreed to support food stamps. The resulting act’s stated purpose was to permit low-income households to “receive a greater share of the nation’s food abundance.”

Even then, the debate over what people should be allowed to buy was fierce. Southern Democrats wanted to ban soft drinks, luxury foods, and luxury frozen foods, though those last two categories were never sufficiently defined. The Senate kept the definition of eligible food broad, rejecting the House Agriculture Committee’s attempt to prohibit soft drinks because it would cause “insurmountable administrative problems” for retailers. That argument, administrative inconvenience, would echo through the decades every time someone tried to restrict or expand the program.

Hot food was excluded from the start for a straightforward reason: the program was built around the assumption that recipients would shop for groceries and cook at home. A hot meal at a deli counter was considered the equivalent of dining at a restaurant, a luxury outside the program’s scope. In 1964, when most American families, including poor ones, did cook from scratch most nights of the week, this logic held.

The Rule That Refused to Evolve

The 1977 Food Stamp Act made significant reforms, eliminating the purchase requirement that had forced recipients to buy their stamps with cash, a barrier that had shut out many of the poorest families. But the hot food exclusion stayed. It was formally codified in the 2008 Food and Nutrition Act, which locked in the language that still governs SNAP today: benefits may be used for “any food or food product for home consumption except alcohol, tobacco, hot foods, or hot food products ready for immediate consumption.”

The government did eventually acknowledge that some people simply cannot cook for themselves. The Restaurant Meals Program, a limited exception, allows elderly adults aged 60 and older, disabled individuals, and homeless households to use SNAP benefits to purchase prepared meals at participating restaurants. It is a compassionate carve-out, but it is available only in select states, covers a narrow slice of the SNAP population, and does nothing for the working parent, the overnight-shift employee, or the family without reliable transportation to a participating restaurant.

Meanwhile, the population that needs relief has grown far beyond what the 1964 architects imagined. According to a 2021 USDA report, 30 percent of SNAP recipients cite lack of time as a barrier to home cooking. Another 15 percent point to physical disability, and 11 percent report not having adequate kitchen equipment. Many recipients live in transitional housing or on the street, places without a stove or refrigerator. The rule designed to encourage home cooking is now simply steering people toward cold food or no food.

The State-Level Wave Making Things Worse

While the federal hot food ban has remained frozen in place, a parallel and equally troubling wave of new restrictions is sweeping the states. As of May 2026, 22 states have received federal waivers to ban specific food items from SNAP, with restrictions on soda, candy, energy drinks, and other so-called junk foods taking effect on a rolling basis. Indiana, Iowa, Nebraska, Utah, and West Virginia were among the first states to implement these waivers beginning January 1, 2026.

Health and nutrition experts have raised serious concerns. Anand Parekh, a physician and chief policy officer at the University of Michigan School of Public Health, has noted that food restrictions do not solve “the two fundamental problems, which is healthy food in this country is not affordable and unhealthy food is cheap and ubiquitous.” Previous USDA research concluded that restrictions would be costly and complicated to implement and might not change recipients’ buying habits or reduce health outcomes at all.

The retail industry is equally alarmed. The National Retail Federation has warned of longer checkout lines and more customer complaints as point-of-sale systems struggle to navigate inconsistent, state-by-state restricted food lists. Kate Bauer, a nutrition science expert at the University of Michigan, described it plainly as “a disaster waiting to happen of people trying to buy food and being rejected.”

There is also a deeper contradiction worth naming. Health Secretary RFK Jr. has built his public health platform around the idea that Americans deserve better, cleaner food. But the restrictions being approved under his watch apply only to people on SNAP. Wealthy Americans can fill their carts with soda and candy without government interference. Only the poor are subjected to nutritional supervision at the register. Selective health crusades aimed only at the vulnerable are not public health. They are paternalism.

What Honest Reform Looks Like

Congress should pass the Hot Foods Act in full, not just the rotisserie chicken carve-out that cleared the House in April 2026. Limiting hot food eligibility to one type of chicken is better than nothing, but it is not equity. Hot prepared foods at authorized grocery retailers should be broadly eligible for all SNAP recipients, not just those lucky enough to live near a Costco.

The USDA should also push back on state junk food waivers that lack strong evidence of improving nutrition outcomes. Good intentions are not the same as good policy, and the administrative chaos these waivers are creating at checkout counters across the country is a predictable consequence of rushing ideologically driven restrictions into law without adequate infrastructure or research to support them.

The rule at the center of this debate was built for a version of America that no longer exists: one where poor families had time to cook, kitchens to cook in, and wages high enough to make grocery shopping straightforward. That America is gone. The question now is whether Congress has the will to update the law to match the country it actually governs.

The Two-Tiered Tax System

The federal tax code of the United States is a progressive tax system wherein higher-income individuals shoulder a greater share of the tax burden than lower- and middle-income taxpayers. It is purported to tax those with greater ability to pay at higher rates whilst providing relief to low-income households, however, evidence from the past year displays a story incongruent with the progressivity purported to exist in the tax code.

When Congress passed the One Big Beautiful Bill Act, H.R. 1, 119th Congress (2025) and signed it into law on July 4, 2025, it made the most sweeping regressive changes to federal tax policy since the 2017 Tax Cuts and Jobs Act. The distributional effects of the 2025 budget reconciliation act breaks along income lines, according to the Tax Policy Center, the law reduces taxes on average by about $2,900.00 in 2026 with average tax cuts being generally larger as a percentage of after-tax income for higher income households than for lower income households. Nearly 60% of the tax benefits flow to households in the top quintile, that is, those earning $217,000.00 or more (Distributional Effects of the Tax Provisions in the 2025 Budget Reconciliation Act). Meanwhile, the lowest income households making less than about $35,000.00 receive an average cut of roughly $160.00, less than 1 percent of their after-tax income. Middle-income households, who will make between about $67,000.00 and $119,000.00, would pay an average of $1,850.00 less, or 2.4 percent of their after-tax income (TPC Finds Final House Budget Bill Cuts Average Taxes By $2,900, Mostly For High-Income Households). This disparity is not a rounding error but rather the very architecture of the law. By contrast, those making between $460,000.00 and $1.1 million would receive an average tax cut of about $21,000.00, or 4.3 percent of their after-tax income (TPC Finds Final House Budget Bill Cuts Average Taxes By $2,900, Mostly For High-Income Households). The highest-earning 0.1 percent of households, those making $5,000,000.00 or more, would receive average tax cuts of almost $300,000.00 in 2026, about 3.3 percent of their after-tax income (TPC Finds Final House Budget Bill Cuts Average Taxes By $2,900, Mostly For High-Income Households).

When tariffs, taxes that add to the cost of imported goods, are added to the picture, the divergence becomes even more stark as tariffs will reduce after-tax incomes for all income groups. The Tax Policy Center estimated that tariffs announced by the Trump administration through December 4, 2025, will impose an average burden of about $1,050.00 per tax unit (or household) in 2026 (TPC Tariff Tracker | Tax Policy Center). The combined distributional impact of the H.R.1 and the current administration’s 2025 tariff increases finds that those in the bottom income decile will see an average reduction in after-tax-and-transfer income of about $2,700.00, whereas those in the top decile will see an increase of nearly $8,000.00 on average (Combined Distributional Effects of the One Big Beautiful Bill Act and on Tariffs). Tax cuts skewed towards the most wealthy, tariffs absorbed by consumers, and spending cuts to Medicaid and food assistance resulted in a tax increase for the average American in all income groups except the richest 5 percent (State-by-State Estimates of the First Year of Trump’s Tax Policies: All But the Richest Americans Face Higher Taxes). This is the two-tiered tax system wherein one set of rules apply to those who invest and accumulate wealth and another set of rules is applied to those who clock in, pay rent, and tread water to stay afloat financially.

Tax Credits That Actually Reach Working Families and their Limits Under New Law

For low- and moderate-income families, the Earned Income Tax Credit (EITC) and the Child Tax Credit (CTC) are two of the most impactful tax benefits which reduce poverty in the United States (2025-CTC-and-EITC-policy-brief.pdf). The EITC, created by the Tax Reduction Act of 1975 (TRA75; P.L. 94-12) originally provided a credit in the amount of 10% of the first $4,000.00 in wages a worker earned. EITC is fully refundable and phases in with earned income, meaning it rewards work dollar for dollar up to a maximum benefit, then tapers as income rises. Working families with children earning below roughly $50,434.00 to $68,675.00 annually may be eligible for the federal EITC when filing for tax year 2025, and during the 2022 tax year the average EITC for a family with children was $3,338.00 (The Earned Income Tax Credit). Though, its reach falls just shy of fully grasping single adults since a single adult, without children, working full-time, at the federal minimum wage, for tax year 2025 would have an EITC amounting to just $308.00. This is too small to offset federal income and payroll taxes not to mention state income taxes. Under current law roughly 6 million working adults without children will be taxed into, or deeper into, poverty in 2026 (The Earned Income Tax Credit).

The Child Tax Credit, created by The Taxpayer Relief Act of 1997 (TRA97; P.L. 105-34) and enacted a nonrefundable $400.00 per child tax credit tells a parallel story of promise constrained by design. H.R.1 increased the maximum CTC to $2,200.00 per child for 2025 and 2026 but retained the partial refundability structure that systematically excludes the poorest households. While the richest 20 percent of Americans will receive the largest benefit from the CTC change among any income group in 2026, the poorest 20 percent will receive nothing, because 99% of children in the poorest fifth of households will receive a reduced credit or no credit at all due to income restrictions left intact by H.R.1 (The Child Tax Credit Leaves Out Millions of Children in 2026. H.R.1’s average CTC benefit is $0.00 for the poorest fifth of families with children and $240.00 for the middle fifth (State-by-State Estimates of the First Year of Trump’s Tax Policies: All But the Richest Americans Face Higher Taxes).

The Refund Economy: What a Tax Refund Actually Means for Families Right Now

Three weeks after Tax Day, families across the country are not thinking in abstractions. They are checking their balances daily and stretching every dollar, knowing the refund hasn’t hit yet. Bills don’t wait: rent is due, prescriptions need refilling, the fridge needs restocking. A tax refund is not a bonus but rather the rightful return of money that was already earned and already withheld. The “refund economy” refers to the structured financial cycle in which millions of low- and moderate-income households anchor major purchases, debt payments, and key consumption decisions to their annual tax refund.

For millions of low- and moderate-income households, it is the single largest cash event of the year, and its timing determines whether a family enters the spring on stable footing or deeper in arrears. The EITC alone delivered approximately $64 billion to 23 million working families in 2024, according to ITEP’s state EITC analysis, and together with the federal Child Tax Credit, it lifted an estimated 6.8 million people out of poverty in 2024 (State Earned Income Tax Credits Support Families and Workers in 2025). That money does not sit idle in a high yield savings account or make it to an investment account but is earmarked to pay landlords, cover car repairs essential to maintaining employment, and funds the school supplies and medical copays that fall through paycycles. The growing importance of the refund as a financial stabilizer is a symptom of a broader problem: wages have not kept pace with the cost of housing, healthcare, or food, and the tax code has become one of the few mechanisms through which working families periodically reclaim economic ground. When credit design leaves out the lowest-income households — as H.R.1’s CTC does —families must wait for a lump sum that arrives weeks after the bills did. That gap is where debt accumulates, where evictions begin, and where the compounding disadvantages of poverty deepen.

The EITC, like many tax credits targeted to low‑income workers, functions as an imperfect makeshift savings tool providing a temporary moment of financial slack at tax time. Credits do not create sustained reserves that households can draw on throughout the remaining eleven months of the year. As a result, even a modest financial shock such as an unexpected car repair, a medical bill, or a reduction in work hours can set off a chain of events that pushes a low-income family into deeper financial instability or poverty. The Becker Friedman Institute for Economics at the University of Chicago has further documented that refund uncertainty —uncertainty about the amount or timing of the annual refund that persists even after other income uncertainty is resolved—itself distorts recipients’ consumption and savings choices and produces welfare losses among EITC filers.(Caldwell, Nelson, and Waldinger, University of Chicago Becker Friedman Institute, 2024).

Growing American Dependence on the Annual Refund

The dependence of American households on the annual tax refund has deepened measurably in recent years, tracking the widening gap between wages and the cost of basic necessities. The Federal Reserve Board of Governors’ research division, using anonymized daily state-level spending data, found that EITC recipients spend approximately 15 cents out of each dollar of their total refund at retail stores and restaurants within just two weeks of receipt. A spending response comparable in magnitude to the 2008 federal economic stimulus payments, and one that is measurably disrupted when refund timing is delayed (Federal Reserve Board, High-Frequency Spending Responses to the Earned Income Tax Credit, 2018). The very same study found that even a two-week delay in refund issuance — such as what occurred in 2017 under the Protecting Americans from Tax Hikes (PATH) Act — produced a noticeable suppression of national retail spending followed by a surge when refunds were finally released. Demonstrating how entirely ordinary household spending patterns in low-income communities depend on the predictability of refund disbursement timing. The Center on Budget and Policy Priorities further confirmed that surveys of EITC recipients consistently show most intend to use their refunds to invest in economic and social mobility — including savings for housing, children’s education, and transportation expenses vital to employment — as well as making ends meet in the short term (Center on Budget and Policy Priorities, EITC and Child Tax Credit Promote Work, Reduce Poverty, and Support Children’s Development, 2015).

Call to Action: Three Things You Can Ask Your Legislators to Do Right Now

The tax code is not an abstract force. It is written, revised, and voted on by people who answer to us. And there are clear, immediate steps they can take to make it a fairer and more effective instrument of economic justice. Tax credits can be used to support households with a range of other necessary expenses, from education, to rent, energy assistance, and more. The following are specific, evidence-backed actions you can demand from your federal and state legislators today, by phone, email, or in person.

1. Restore a fully refundable Child Tax Credit and reinstate monthly payments.

Contact your members of Congress and urge them to support legislation that makes the Child Tax Credit fully refundable — so the lowest-income families are no longer excluded — and restores the monthly advance payments implemented under the American Rescue Plan Act of 2021. That policy cut child poverty nearly in half during its brief expansion, largely because it delivered support when families actually needed it — not as a delayed lump sum at tax time.

2. Expand the Earned Income Tax Credit for workers without children.

Ask your legislators to back proposals that significantly increase the Earned Income Tax Credit for workers not raising children at home. Urge lawmakers to significantly increase the EITC for workers who are not raising children at home, a group that still receives far smaller benefits than families with children. This is one of the most effective, bipartisan tools for boosting incomes among low-wage workers, yet it remains underpowered where the need is greatest.

3. Bring independent, evidence-based voices into the policymaking process.

Urge lawmakers to formally engage with organizations such as the Center on Budget and Policy Priorities and the Institute on Taxation and Economic Policy through hearings, briefings, and legislative consultations.

Mothers are our first protectors, but their shield extends far beyond the cradle. When injustice arrives, mothers are the first to meet it. They are the quiet architects of movements, sustaining leaders like Martin Luther King Jr. and carrying the torch of his dream long after his voice was silenced. They are the frontline warriors in the streets, answering the primal, agonizing cry of a man calling for his "momma" as the world watched him die.

A mother is a rare and formidable alchemy: grace and fire, hurricane and warrior, the chaos of the storm and its calm center, all bound by fierce pride. They do more than bear pain; they absorb it. They carry it. They open their doors to the discarded, providing sanctuary to those who are cast out for the "crime" of being themselves.
This Mother’s Day is more than sentimental remembrance. It is a tribute to the lineage of women who fought so that the girls following them could walk with lighter steps and stronger hearts.

History often fears a mother’s resolve. They lead revolutions even when labeled as threats. They educate generations, passing down the embers of change, teaching us to tend the fire until we are ready to stoke our own flames. They fight until the very last breath.

In 1902, Mary Harris Jones was branded "the most dangerous woman in America" simply because she organized mine workers and their families against exploitation. Similarly, Black mothers, the perennial trailblazers against systemic injustice, are frequently shamed and stereotyped as "angry" or "irrational." These labels are a thin veil used to dehumanize them, an attempt to mask what they truly are: exceptionally strong and passionately dedicated.

A mother will place her body in the line of fire. She will stand before snarling dogs and high-pressure hoses to protect all children, not just her own.

As we enter this Mother’s Day weekend, recognize mothers as the original organizers. They work past exhaustion and keep working. This weekend, thank a mother for her fierce love, her relentless dedication, and for being the warrior the world has always needed.

What do St. Paul, Kansas, and New York City have in common? Public grocery stores.

On the campaign trail, Mayor Zohran Mamdani proposed a bold initiative to establish five city-owned grocery stores, one in each borough, to combat a staggering 66% spike in local food costs over the last decade. Within his first 100 days, he has moved this from a stump speech to a site plan, announcing La Marqueta in East Harlem as the flagship location.

Critics may dismiss the plan as “radical,” but the residents of St. Paul, Kansas, have a different word for it: essential. Since 2008, their municipal market has proven that when the private sector retreats, cities can step in to treat food access like a public utility, no different than water or electricity.

The St. Paul model was born of necessity. After the town spent 20 years without a grocer, newly elected Mayor Rick Giefer conducted a feasibility study and secured a $400,000 no-interest loan from the USDA. “We almost lost our high school,” Giefer recalled. “We realized that to draw people to town, we needed a food source other than a convenience store.” Other towns had the same idea. Baldwin, Florida; Erie, Kansas; and Hay Springs, Nebraska have all experimented with public grocery stores. But while St. Paul thrives, others have faltered. In Baldwin and Erie, the stores recently shuttered, leaving residents back in the food deserts they tried to escape.

So why did they fail, even with the city covering rent and waiving taxes? The answer lies in a forgotten 1936 federal law: the Robinson-Patman Act. Originally passed to prevent massive chains from using their size to bully suppliers into lower prices, the Act ensured a level playing field for “mom and pop” shops. Enforcement, however, effectively ended in 1977 under the Carter administration. The result was the “Waterbed Effect”: as big-box retailers demanded deep discounts on one side of the market, prices rose for everyone else to compensate.

Baldwin and Erie’s public stores were, in effect, “chains of one.” Without federal protection, they were forced to pay significantly higher wholesale prices than their corporate competitors, and even with zero rent, they couldn’t match the shelf prices of a Walmart miles away.

This is the trap Mayor Mamdani is trying to avoid. Rather than five isolated stores vulnerable to the same squeeze, NYC plans to leverage city-owned warehouses to purchase food in massive volumes for the entire network. By acting as its own wholesaler, the city can mimic the purchasing leverage of a major chain, securing lower prices from distributors and passing those savings on to shoppers.

The lesson from Kansas to East Harlem is the same: food access doesn’t have to be left to the market. These projects represent a refusal to let marginalized communities go hungry in the pursuit of corporate profit, and by treating food as a public good, these cities are doing more than just selling milk. They are reviving a national debate on market concentration and demanding a fair shake for the communities that need it most.

“Tell me. What makes you hesitate?”

This is often my first response when a patient states they don’t want treatment. I ask it quietly, while putting my stethoscope down on the desk and sliding into a chair next to them. For a few moments, a busy day freezes and slows down to a halt. This is the moment when I have the opportunity to judge, to criticize. Instead, I try to make the crucial choice to lean in.

I often get asked, “How do you handle it? Don’t you get angry when you know the treatment a patient needs, when you know what could help them, and they refuse? Don’t you get upset when a patient refuses vaccines?”

I can honestly answer that I don’t. I don’t get angry. In a country that feels increasingly polarized, a country that shows little trust in its medical institutions, a country with horribly disparate outcomes compared to equally wealthy nations, a patient’s lack of trust does not irritate me. Even after working during the COVID-19 pandemic and watching the lives lost, the anger has never sat right with me. Instead, it just breaks my heart.

Hesitancy and refusal often start with a story, if we slow down to listen to it. Something like:

“Well, when my mother took it. . .”

“I saw what happened to my sister. . .”

“I’ve heard these reports, and I’m afraid.”

Whether or not these stories truly relate to the treatment being refused is not up to me to decide. What is up to me is to listen. To hear the fear, the hurt, the loss that surrounds such a tense issue. To validate the pain.

Sometimes, hesitancy starts from a place of anger, which can be more difficult to manage. But, I think, if we take the moment to listen and dig a little deeper, we can hear the same themes: themes of fear. Themes of feeling uncared for. Themes of hurt. The fear, though masked with barbs of rage, is no less valid. It reflects a wound.

Treatment refusal and vaccine hesitancy is not a character flaw to be corrected. Doubt in standards of practice reflects a deeper instability in our system as a society. It represents the difficult question we often leave unasked, questions of how we know truth in a world inundated with fake news and misinformation.

I speak this all as a nurse who faces patient hesitancy every day. I speak also as a patient myself, with a life-long disability. I have experienced the best of evidence-based care and compassion and the worst of greed and inadequate practice. I have experienced being rushed to take a medication before the consequences were explained to me.

As clinicians, we should be the most skeptical. We should be the first to read an article and analyze its flaws. We should be the first to avoid jumping to unproven treatments. We know best of anyone the pressures we face in clinical practice. We know painfully well the failures our profession has propagated, such as during the opioid crisis. We know the authority we hold. We should not see hesitancy as a shock or as a nuisance, but as a place to have a hard conversation about life, autonomy, and access to information.

What does all of this mean? Today, during World Immunization Week, this is a call to practice radical empathy and humility. Whether you work in healthcare, public policy, or, like many of us, you spend too much time responding to comments online, this is a call to share truth and information with patience. This is not a call to forsake science or patient education, but it is a call to slow down and listen. It is a call to recognize the person across from you, not as stupid or uneducated, but as a whole person. Those of us who work in anti-poverty work want so desperately to alleviate the sufferings we see. It can break us down when we see misinformation stopping that, when we see news reports and practices so contrary to what we know as best evidence.

But we are not saviors.

We are trying to do the best for our loved ones and communities. No matter how much education we give, we cannot convince everyone of everything. What we can do is listen. And if that is the only healing or respite someone receives from me, if they walk away still refusing treatment despite all my education, I have to accept that.

I have seen patients die from refusing treatment.

I have seen others live from accepting it.

I have loved both the same.

I have wept with both the same.

This World Immunization Week, I encourage you to have tough conversations about the benefits of vaccinations and listen earnestly to those who disagree with you. I challenge you to have hard conversations knowing that you may never see tangible change. If there are a few things the human race deserves, it is knowledge, autonomy, and a listening ear.

The obligation runs in both directions. Parents owe their communities the protection that comes from vaccination. But communities — and the governments that represent them — owe parents something too. They owe us the truth. They owe us the science, delivered clearly and without political interference. They owe us the clinics, the community health workers, the cold storage chains, the surveillance systems — all the invisible infrastructure that makes a shot available to a child who needs it. And they owe us the global investment that ensures the disease does not arrive from abroad in the first place.

Right now that obligation is being defaulted on at every level. The United States contributed zero dollars to Gavi, the Vaccine Alliance, for the 2026–2030 funding cycle — leaving an alliance that has vaccinated more than 1.2 billion children and prevented more than 20 million deaths nearly $3 billion short of what it needs to protect 500 million more. For every dollar invested in immunization in Gavi-supported countries, $21 are returned in healthcare costs, lost wages, and lost productivity prevented. The United States has decided that return on investment is no longer worth making. That is not fiscal responsibility. That is a choice — made on behalf of all of us, without asking us — to let children die from diseases that cost less than a cup of coffee per dose to prevent.

I am a mother and a journalist, not a lobbyist. But I have spent five days laying out what is at stake and I am not willing to end this series without saying plainly what I believe needs to happen. These are not partisan positions. They are the minimum requirements of a society that takes seriously its obligation to protect its children — all of them, not just the ones born into the right zip code or the right country.

What Has Already Been Proven

I do not want to end this series in despair. Not because the situation does not warrant it — it does — but because despair is not useful and the evidence does not support it as a final conclusion.

Here is what the evidence actually shows. In Africa, nearly 20 million measles deaths have been averted since 2000 through sustained vaccination investment. Child mortality in 78 lower-income countries has been cut in half since Gavi was founded. Smallpox — which killed 300 million people in the 20th century — was eradicated from the entire planet through coordinated global vaccination. Polio, which paralyzed tens of thousands of American children annually in the early 1950s, has been eliminated from all but two countries on earth. These are not projections or promises. They are things that already happened. Things that human beings built together, across borders and political differences and resource constraints, because they decided that children dying from preventable diseases was not acceptable.

That decision is still available to us. It has not expired. It does not require a new discovery or a new technology or a new vaccine. It requires only the will to maintain what has already been built — and the courage to demand that the people in power stop dismantling it.

Where This Started

Five days ago I told you about a mother in a waiting room who would not let anyone give her daughter a shot. I told you she was scared. I told you she was wrong. I told you she changed.

That mother was me.

My daughter is vaccinated now. She is healthy and stubborn and wonderful and every single day I am grateful — not just that she is protected, but that I found my way to understanding what her protection actually required of me. It required more than love. Love was never the question. Love was always present. What was missing was the understanding that love, on its own, is not enough to protect a child from a disease that a needle can prevent. That love has to be accompanied by action. And sometimes that action is uncomfortable. Sometimes it looks like holding your child’s arm still while she cries. Sometimes it looks like calling your senator. Sometimes it looks like changing your mind in public and refusing to be embarrassed about it.

I changed my mind. I am telling you about it because I think the changing of minds is the most important thing that can happen right now — more important than any policy, more important than any funding decision, more important than any headline. Because policies follow people. Funding follows political will. And political will follows the quiet, private decisions of millions of parents who decide, one by one, that their child’s protection is inseparable from every other child’s protection.

My daughter does not know Paul Alexander’s name. She does not know about Burghardt Du Bois or the O’Marra family or the iron lung wards or the diphtheria charts with their careful handwritten notes about membranes and difficult breathing. She does not know about the mother calling the closed clinic in Dallas County or the CDC scientist locked out of her system before she could say goodbye to the work. She does not know any of this because she does not need to. She is a child. Her job is to be a child.

Our job — mine, yours, the government’s, the global community’s — is to make sure she stays one, and to make sure other children get to stay children too.

They work when we use them. They work when we fund them. They work when we fight for the systems that deliver them to every child — not just the children born into countries and communities and zip codes where access is a given. They work when we treat immunization not as a personal choice but as a collective covenant — the agreement we make with each other that no child should die from a disease that a needle can prevent.

That covenant is under threat right now in ways it has not been in decades. The threat is real. The consequences are already visible — in the measles cases in 33 states, in the closed clinics, in the terminated scientists, in the zero dollars sent to the children who needed them most.

But the covenant itself has not been broken. Not yet. It is still possible to honor it. It is still possible to rebuild what has been dismantled, to restore what has been cut, to re-engage with the partners we have abandoned and recommit to the children we have left behind.

That is what we owe each other. Not perfection. Not the absence of fear or doubt or hesitation. Just the willingness to keep showing up — for our children, for each other’s children, for the children whose names we will never know but whose lives depend, in some small and specific way, on the choices we make in the rooms where they are not present to speak for themselves.

His name was Paul Alexander. He was six years old when polio came for him in Dallas, Texas, in the summer of 1952. Within a week, the disease had taken his ability to breathe. He would spend the next 72 years inside an iron lung — a seven-foot metal cylinder that breathed for him by manipulating the pressure around his chest. He died on March 11, 2024. He was 78 years old. He had outlived almost everyone who remembered what it meant to need a machine to stay alive.

He is not ancient history. He is a man who was alive two years ago. A man who went to college, earned a law degree, wrote a memoir by holding a rod in his mouth and tapping keys one letter at a time. A man who built a life of extraordinary determination inside and around the constraints of a disease that no child in the United States should ever contract again.

Alexander contracted polio before the vaccine existed. The children contracting measles in South Carolina right now — 997 confirmed cases in a single county as of this spring — did not have that excuse. The vaccine exists. It has existed since 1963. It is safe, effective, and available. And yet here we are.

I needed to understand what I had been refusing to vaccinate my daughter against. I needed to make the disease real — not as a statistic, not as a historical footnote, but as something that happened to a specific child in a specific city in a specific summer. And when I found him, I found something I was not expecting. I found a child who reminded me of mine. Curious. Stubborn. Full of life. Reduced in an instant by something a needle could have prevented.

What We Chose to Forget

I did not know most of what follows until I went looking for it. I am an advocate, a journalist, and a mother, not a historian of medicine. But I have spent the last several weeks reading — in archives, in medical records, in the personal accounts of parents who watched their children die from diseases that now have vaccines — and I want to share what I found with the same directness with which I found it. Some of this is difficult to read. It should be.

Today: Polio has been eliminated from the United States since 1979. In 2022 a young unvaccinated man in New York contracted paralytic polio — the first U.S. case in nearly a decade. Polio remains endemic in Pakistan and Afghanistan. It is one international flight away from any unvaccinated community in the world.

Today: Diphtheria has been nearly eliminated in vaccinating nations. Between 1980 and 2004, just 57 cases were reported in the United States. But after the Soviet Union collapsed in 1991 and vaccination rates fell, 200,000 cases and 5,000 deaths were recorded in the former Soviet states within seven years. The lesson — that elimination is not eradication, and that political disruption can undo decades of protection in a single generation — is one we are currently choosing to ignore.

Today: Smallpox officially exists only in two secure laboratory samples — one in the United States, one in Russia. The vaccination programs that once protected humanity against it ended after eradication. A new generation has no immunity. If smallpox were ever to escape containment — through accident, misuse, or bioterrorism — it would find a world almost entirely unprotected. That is not a projection designed to frighten. It is a documented biosecurity concern that scientists have raised for decades.

Today: Whooping cough has returned. In 2024 the United States recorded tens of thousands of cases — the highest levels in decades. Infants too young to be fully vaccinated are the most vulnerable. The resurgence is directly connected to declining vaccination rates in the communities surrounding them. A two-month-old cannot protect herself. She depends entirely on the immunity of the people around her. When that immunity drops, she has nothing.

The Parents We Do Not Remember

Here is what I keep coming back to. Every child who died from these diseases had a parent who loved them. Not differently than you love yours. Not less fiercely or less completely than I love mine. They loved in the same way. The same sitting-up-at-night way. The same watching-them-sleep way. The same I-would-do-anything way.

They did not have the option I had in that waiting room. They could not hesitate. They could not switch pediatricians. They could not take a few more months to feel certain. The disease arrived whether they were ready or not. And when it arrived, they had nothing to offer their child but presence — and prayer — and the terrible helplessness of watching something they could not stop take someone they could not live without.

The O’Marra family of Kansas lost eight of their nine children to a particularly severe strain of diphtheria in 1903. Eight children. One summer. One family. One disease that now has a vaccine that costs less than a cup of coffee.

I think about those parents constantly now. I think about them when I see someone post about not vaccinating. I think about them when I read about a politician cutting the funding that keeps those vaccines accessible. I think about them and I think: they would have given anything for what we have. They did give everything. They gave their children. And we are choosing, in full knowledge of what that cost, to let it happen again.

What Forgetting Actually Costs

The diseases in this piece did not disappear because they became less dangerous. They disappeared because we built systems — laboratories, cold storage chains, vaccination programs, public health infrastructure, global surveillance networks — that kept them from reaching our children. The diseases are still there. Polio circulates in Pakistan and Afghanistan. Diphtheria resurged in the former Soviet states when vaccination rates fell. Measles is back in 33 American states right now. Whooping cough has returned to communities where vaccination rates have dropped below the threshold that protects the most vulnerable.

We did not defeat these diseases. We contained them.

And containment requires maintenance. It requires funding. It requires the political will to keep doing the unglamorous, invisible work of public health even when the diseases are not visible — especially when the diseases are not visible. Because the moment they become visible again, children are already sick. And some of them will not recover.

Alexander lived outside the iron lung in every way that mattered. He built a life. He earned a law degree. He wrote a book. He made TikTok videos that gathered 330,000 followers who watched a man in a metal cylinder explain what it meant to be alive. He was extraordinary. And he should never have needed to be.

The vaccine that would have protected him was developed three years after he contracted polio. Three years. He missed it by three years. He spent 72 years in an iron lung because of three years.

The children getting measles in South Carolina right now are not missing the vaccine by three years. They are missing it in a world where it already exists, inside a public health system so defunded that it can no longer reliably reach the communities that need it most.

The vaccine Paul Alexander never had now exists. That is what makes the return of these diseases feel less like fate and more like failure.

That is a different kind of tragedy. Not the tragedy of a vaccine that did not yet exist, but the tragedy of a vaccine that exists, that works, that is available — and that we are choosing, for reasons that will not survive the judgment of history, not to use.

The O’Marra family did not have the choice we have. They buried their children in the ground and they grieved and they went on living in a world that offered them nothing. Public health infrastructure is invisible until it is gone. By the time you notice it is missing, someone is already sick. This is what collapse looks like before the obituaries are written.

Nobody thinks about the water until the tap runs dry. Nobody thinks about the electricity until the lights go out. And nobody — not one parent in a thousand — thinks about the public health infrastructure that protects their child until the clinic is closed, the phone rings out, and the appointment that should have taken fifteen minutes becomes a three-county search for a nurse who still has the vaccine in stock.

This is the part no one sees.

Nobody sees the epidemiologist who catches the outbreak before it becomes news.
Nobody notices the cold storage chain that gets the vaccine to the rural clinic on time.
Nobody mourns the surveillance system quietly watching seventeen counties for signs of measles.

Until it stops.

The hardest story to tell is the one about infrastructure — because the thing being destroyed is invisible until it is gone. Public health does not announce itself when it is working. It prevents the headline. It interrupts the pattern. It closes the gap before anyone realizes there was one.

And that invisibility is exactly what makes it so easy to dismantle.

These numbers are real.

But numbers are not what collapse feels like.

What collapse feels like is a Tuesday morning. Let me tell you about one.

Her Name Was Not in the News

She had worked at the CDC for more than a decade. She was an epidemiologist — one of the disease detectives the agency has deployed since the 1950s to get ahead of outbreaks before they become crises. Her job, on most days, did not make the news. That was the point. She watched. She tracked. She noticed when three children from different zip codes presented with similar symptoms in the same week and she opened an investigation before the fourth child got sick. She caught things. Quietly. Routinely. In ways that never produced a headline because the headline was the thing she prevented.

On Tuesday, April 1, 2025, she received a termination notice.

She was not alone. More than 2,400 CDC employees received notices that day — nearly one-fifth of the agency’s workforce. Disease detectives. Outbreak forecasters. Laboratory scientists whose work was unlike anything being done anywhere else in the world. Contact tracers. Cold storage specialists. The people who arranged travel and security for CDC scientists responding to international outbreaks. Even the IT staff — so many of them were let go at once that when some employees who had been terminated in error tried to get back into their systems, there was no one left to fix the problem.

One CDC epidemiologist still employed at the agency told reporters: the colleagues who were fired were locked out of network access so quickly they could not pass along critical information to the people still there to pick up their work. Another staffer described the environment plainly. “Outbreak responses are very stressful when you have all the available resources,” she said. “To be on outbreak response now in this climate is a whole new level of complexity and stress.”

There was a Vessel Sanitation Program that inspected cruise ships for disease outbreaks — not funded by taxpayer dollars, but by fees from the cruise lines themselves. Every full-time employee was laid off anyway, including the epidemiologist leading outbreak response, at the exact moment the country was battling a record surge of norovirus with at least a dozen documented outbreaks on cruise ships. One epidemiologist remained. They were still in the early stages of their training.

These are not abstractions. These are the specific absences that allowed specific diseases to spread further than they should have — quietly, without headlines, exactly the way the system was designed to prevent.

It was an ordinary Tuesday. The kind of Tuesday I did not appreciate until I started reporting this piece. I called the pediatric clinic. Someone answered. The appointment was available. The vaccine was in stock. The nurse was trained and the cold storage was working and the records system was connected to the state immunization registry and the county health department had a surveillance team watching for patterns that might signal an outbreak — and all of it, every invisible layer of it, worked. My daughter was protected before she walked back out the door. I did not think about any of that. I just drove home.

That ordinary Tuesday is being dismantled. Quietly, specifically, with line-item precision. And the people doing the dismantling are counting on the fact that most of us will not notice until a child is sick — by which point the damage is already compounding in ways that cannot be quickly undone.

The Places Where It Is Already Gone

These are not projections. These are not warnings about what might happen. These are places where the system has already been cut, closed, or suspended — and the specific human consequences of each decision.

I sit inside systems like this every day.

Not in a lab. Not in a federal building.
But at the point where people meet the systems that are supposed to protect them.

I know what a Tuesday like that feels like.

In my work, I sit at the intersection of people and systems every single day. I hear what happens when someone reaches out for help and expects something on the other end — a response, a resource, a pathway forward.

And sometimes, what’s on the other end is… nothing.

Not because no one cares.
Not because the need isn’t real.
But because the system that was supposed to respond has been stretched, reduced, or quietly removed.

I’ve seen what it looks like when something that should be simple becomes complicated. When something that should be available isn’t. When something that used to work no longer does — and no one can quite explain when it stopped.

There’s no announcement when that happens. No moment where someone says, “This is the point where the system failed.”

It just becomes harder. Slower. Less reliable. Until one day, it’s not there in the way it used to be at all.

That’s what collapse feels like.

Who Bears the Weight When the System Fails

Public health infrastructure does not fail equally. When a vaccine clinic closes in an affluent suburb, families with cars, flexible schedules, and private insurance find another provider. They call their pediatrician. They drive twenty miles. The protection continues, imperfectly but sufficiently.

When a vaccine clinic closes in a low-income neighborhood, in a rural county, in an immigrant community where the health worker who spoke the language has just been laid off — the protection does not continue. It simply stops. The families who depended on that clinic do not have the resources to navigate the alternatives. Their children fall behind on vaccinations not because they do not want protection but because the infrastructure that delivered protection to their doorstep has been dismantled. When federal funding is cut, collapse does not stay in Washington. It moves outward — into states, into counties, into neighborhoods. But it does not stop there either. Because the CDC is not simply an American agency. And what is being dismantled inside those budget lines is not only a domestic public health system. It is the early warning network that the entire world depends on to know what is coming next.

When America Steps Back, the World Goes Dark

The CDC is not simply a domestic agency. It operates in over 60 countries. It is the first call that foreign governments make when a deadly outbreak begins. It is the organization that built the laboratory systems, trained the disease detectives, and established the surveillance networks that watch for the next pandemic before it has a name.

When that organization is weakened — when its staff are terminated, its programs gutted, its scientists barred from speaking to international colleagues — the consequences do not stay in Atlanta. They travel. Quietly. In the same way that diseases do.

Consider what the CDC’s global presence actually meant when it was intact. In 2018, an Ebola outbreak in the Democratic Republic of the Congo went undetected for four months. By the time the world noticed, more than 2,200 people were dead. CDC and its partners responded — and then did something more important than respond. They stayed. They trained. They built. By 2022, when Ebola returned to the DRC, the systems CDC had helped strengthen detected the outbreak within 48 hours. Six deaths. Not 2,200. Forty-eight hours versus four months. That difference is not a coincidence. It is infrastructure.

In 2023, a CDC-trained health worker in Tanzania detected Marburg virus — a disease with a fatality rate that can exceed 80% — during the country’s first-ever outbreak. The worker recognized the signs, triggered an immediate national response, and the outbreak was contained at nine cases with no international spread. Nine cases. No international spread. Because someone was trained to recognize what they were looking at and knew exactly what to do.

In 2025, a dangerous outbreak of pig-borne bacteria began spreading in Hue, Vietnam, during peak tourist season — 1.7 million international visitors in the city at the time. CDC experts worked alongside local partners, launched a full-scale investigation within seven days, and contained the outbreak before a single tourist carried it home on a flight. You did not hear about it. That was the point.

The Global HIV and TB division of CDC’s Global Health Center had seven of its fifteen branches eliminated in a single day — including the entire Maternal and Child Health Branch, whose 22 staff worked to prevent babies from being born with HIV and to keep children with HIV alive on treatment around the world. All 22 were terminated. One scientist on the team noted that with USAID already gutted, her branch had been the only remaining team in PEPFAR with maternal and child health HIV expertise. The State Department had identified their work as critical and lifesaving. HHS cut it anyway.

CDC scientists were barred from speaking to WHO colleagues. When a suspected Ebola outbreak occurred in the DRC, CDC could not make the call to ask what was happening. A CDC fellow who had detected — from testing past samples — that cases of Oropouche virus were going undetected in Florida was prevented by a communications freeze from sharing her response plans with state health officials. Oropouche causes birth defects and fetal deaths similar to Zika. The plans did not get shared. Then the fellow was let go. The summer came. The mosquitoes did not know any of this.

A disease can reach every major city on six continents in 36 hours. The surveillance systems that watch for the pathogen that will make that journey — and try to stop it before it boards the plane — are being dismantled in the name of reducing bureaucratic sprawl. As one former CDC division director put it: surveillance networks take years to build but quickly come crumbling down without support.

The Tuesday That No Longer Exists

Eventually, after the tidal wave from the cuts does its damage across global shores, it finds its way back to its original source. That leads me to think about a mother. I do not know her name. She lives somewhere in Dallas County, or in rural Alabama, or in a community in Georgia where the mobile vaccination van used to show up at the flea market on Saturdays. She has a child who needs a vaccine. She is not hesitant. She is not skeptical. She has no philosophical objection and no ideological reservation. She just wants to protect her child — the same way I wanted to protect mine, even when I was getting in my own way.

She is calling a clinic that no longer answers. She is looking for an appointment at a health department that has laid off the staff who used to schedule it. She is waiting for a mobile vaccination event that was canceled on 24 hours notice because the federal funding that paid for it was taken back on a Tuesday in March 2025 by people in Washington who will never know her name.

That is what the destruction of public health infrastructure looks like before anyone writes the obituary. It looks like a phone that rings out. An appointment that is not available. A van that does not come. A child who goes another month unvaccinated — and another — and another — until the herd immunity threshold in that community quietly drops below the level that protects the children who cannot be vaccinated at all. The immunocompromised child. The infant too young for the vaccine. The child with the allergy. They are protected not by their own immunity but by everyone else’s. And when everyone else’s disappears — they have nothing.

What the Timeline Actually Looks Like

Infrastructure collapse does not announce itself. It happens in stages — each one invisible until the next one makes it legible. Here is the sequence that is already underway.

Stage 1 — Funding cut

Funding cut Federal grants are canceled or clawed back. State health departments receive notice — sometimes 24 hours, sometimes none. Programs stop. Contracts are terminated. Staff are laid off. The machine keeps running on momentum for a few weeks. In Georgia, the email arrived on a Tuesday. The community health workers who had spent years earning the trust of their neighborhoods were told not to come back Wednesday.

Stage 2 — Clinics close Mobile vaccination events are canceled. Community health workers are let go. Appointment availability shrinks. Families in underserved communities begin to lose access first. The gap is invisible from a distance. In Dallas, 50 clinics were canceled while measles was actively spreading. The disease did not receive a cancellation notice.

Stage 3 — Coverage drops Vaccination rates begin to fall in specific zip codes, specific communities, specific counties. The surveillance systems that would detect this are themselves being cut. The gap between what is happening and what is known begins to widen. The United States is here right now. The falling coverage is measurable. The full extent of it is not — because the people doing the measuring have been laid off..

Stage 4 — Herd immunity fractures In communities where coverage falls below the threshold, herd immunity breaks. The most vulnerable — infants, the immunocompromised, those with medical exemptions — lose the protection they depended on from others. They do not know it yet. Neither does anyone else — because the surveillance system that would tell them is no longer fully operational.

Stage 5 — Outbreak A disease finds the gap. Cases appear. The surveillance system that would have caught it early has been cut. The contact tracers who would have contained it have been laid off. The laboratory capacity that would have confirmed it quickly has been reduced. The outbreak grows before anyone fully understands its size. This is not a story about policy disagreements. It is not a debate about the appropriate size of government. It is a story about what public health infrastructure actually is — the specific, unglamorous, irreplaceable work that happens between the moment a disease emerges and the moment it reaches your child — and what it means when that work stops.

The system that protected your child on an ordinary Tuesday did not build itself overnight. It was constructed over decades by people who understood that the cost of prevention is always lower than the cost of catastrophe. What is being dismantled right now will not be rebuilt quickly. The expertise that is walking out the door — the twenty-year epidemiologist, the lab director, the community health worker who spent a decade earning the trust of her neighborhood — does not come back with a budget line. As one Stanford infectious disease specialist put it: you can reinstate a recommendation with a court order, but you cannot reinstate twenty years of accumulated scientific expertise. There is a cruel irony sitting at the center of the global vaccine crisis that does not get named nearly enough. The people who desperately want vaccines cannot afford them. The people who can afford every vaccine ever developed do not want them. And the disease — which reads neither bank statements nor ideology — keeps moving.

This is not a metaphor. It is what I found when I started reporting this piece and could not stop reading. Mothers in sub-Saharan Africa, in rural Bangladesh, in conflict-torn Yemen would hold their children’s arms still without a moment of hesitation. They would walk miles to a clinic. They would say yes — please, give them everything you have. The clinics are not there. The doses are not there. The funding that would have put them there has been redirected, cut, or claimed by nations whose citizens, in growing numbers, are refusing the very thing those mothers are desperate to obtain.

That contradiction did not begin with COVID-19. But COVID-19 exposed it with a clarity and a brutality that the world has not yet fully reckoned with. What follows is an attempt to do that reckoning — honestly, specifically, and with the names attached.

The scale of what happened is difficult to absorb without the specific numbers. When I found them I had to read them twice. Here they are.

The WHO Director-General stood before the world and called it plainly: the planet was on the brink of a catastrophic moral failure.

He was not speaking in abstractions. He was watching doses expire in warehouses in wealthy nations while people died in countries that had been priced, contracted, and maneuvered out of the queue — not through misfortune, but through deliberate policy.

Researchers published what they thought would happen before it happened. I want you to sit with that. Mathematical modeling estimated that when high-income countries preferentially obtained the majority of available doses at the expense of lower-income nations, the result was approximately 900 additional deaths per million people in the global population. Millions of excess deaths. Forecast in advance. Ignored anyway.

The Doctor Who Tried to Warn Us

On September 22, 2017, Dr. Dimie Ogoina — an infectious disease specialist at Niger Delta University in Nigeria — examined an eleven-year-old boy with a strange rash covering his face and body. The boy had already had chickenpox. Dr. Ogoina suspected something he had never personally encountered: monkeypox. He was right. Within weeks, cases were appearing across Nigeria — not in children near animals, as the textbooks described, but in young adult men in modern cities. The virus had changed. It was spreading between people. And it was doing so in ways the scientific community had insisted were impossible.

Dr. Ogoina published his findings. He warned international health bodies. He told anyone who would listen that mpox was no longer the rare, contained disease it had been for fifty years. Nobody of consequence acted. The wealthy world looked at an outbreak in Nigeria and saw someone else’s problem. They filed the reports. They did not fund the surveillance networks, the vaccines, the laboratory infrastructure, or the therapeutics that could have contained it. Genomic research now confirms that mpox had been spreading in human hosts in Nigeria for eight years before it became a global crisis — circulating, evolving, and waiting — while the world looked the other way.

In May 2022, mpox arrived in London — carried by a traveler from Nigeria. Within weeks it had spread to dozens of countries with no travel links to Africa. The WHO declared a global health emergency. Vaccines were fast-tracked. Governments acted with an urgency they had never shown when the disease existed only in Nigerian bodies. That is the pattern. A disease circulates in a lower-income country. Doctors raise the alarm. The funding is not there. The disease evolves, finds a route through international travel, and arrives in a wealthy city. Then it becomes a crisis. Then the money appears.

We cannot continue to neglect epidemics until they reach the Global North. That is not a moral position. It is a biological fact. Viruses do not negotiate travel restrictions.

The Privilege I Did Not See

I have to be honest about something. When I was sitting in that waiting room, wrestling with whether to let a doctor vaccinate my daughter, I was exercising a kind of power I did not recognize as power at the time. I called it doubt. I called it love. I called it caution. All of those things were true. But there was something else underneath them that I did not have the language for then — something I have had to sit with since.

My ability to say no was itself a product of yes. I could refuse the vaccine because the vaccine was there. I could question the schedule because there was a schedule. I could switch pediatricians because there were pediatricians to switch to. I could afford the luxury of hesitancy because the infrastructure of protection surrounded my daughter so completely that I had the freedom to doubt it without immediately endangering her.

That is not available to a mother in the DRC. It is not available to a family in rural Afghanistan or southern Nigeria or northern Yemen.

For those parents, the question is never whether to trust the system. The question is whether the system will show up at all. And increasingly, as funding disappears, as clinics close, as international health organizations are defunded by the nations that built them, the answer is no.

I am not saying that hesitancy is never valid or that questions should never be asked. I asked them. They mattered. What I am saying is that the right to ask them, the time, the access, the safety net that made asking survivable, is not universally available. And when wealthy nations cut the funding that extends that safety net to the rest of the world, they are not just making a budget decision. They are deciding whose children’s lives are worth protecting and whose are worth monitoring from a safe distance until the disease crosses a border that matters.

What COVID Revealed That We Have Not Fixed

COVID-19 did not create vaccine inequity. It illuminated it — with a speed and a scale that made it impossible to look away, though many tried. In Malawi in April 2021, 16,000 AstraZeneca doses arrived with an expiry date two weeks away. There was not enough time to administer them. They were destroyed.

In Haiti, less than 15% of 500,000 donated Moderna doses had been used before they neared expiration — not because Haitians did not want them, but because Haiti was simultaneously managing gang violence, a presidential assassination, a devastating earthquake, and a healthcare system that had never been built to handle a rapid mass vaccination campaign. The donation was not generosity. It was the performance of generosity. The optics of giving without the infrastructure of actually helping.

Meanwhile, the United States, with a population of 332 million, held over 375 million doses. Canada, with a population of 38 million, had contracted enough doses to vaccinate every Canadian citizen more than three times over. These numbers were not secrets. They were reported. Debated. Condemned by international health organizations. And largely unchanged.

Only high-income and upper-middle-income countries achieved the WHO’s target of 70% vaccination coverage by mid-2022. The countries that failed were not failing out of indifference to their own people. They were failing because the doses had been bought out from under them, and the donated remainder arrived too late and in conditions too fragile to deploy. The pandemic did not treat the world equally. The response made the inequality worse — deliberately, measurably, and with full knowledge of the consequences.

We told ourselves during COVID that we were in it together. We were not. The question now is whether we are willing to finally mean it — or whether we will wait, again, for the body count to become undeniable before we act. History suggests we already know the answer. History also suggests that answer can change.

The Call That Cannot Wait

The argument for funding Gavi, the WHO, and global immunization programs is not charity. It is not foreign generosity. It is the most self-interested thing a nation can do — because the alternative is paying for the outbreak instead. And outbreaks are considerably more expensive than the clinics that prevent them. I am a mother. I understand cost-benefit analysis now in a way I did not when I was sitting in that waiting room saying no. Every dollar invested in vaccine access in lower-income countries is a dollar invested in not importing the next outbreak through an international airport. Every surveillance system funded in Nigeria or the DRC is an early warning system for London and New York and every city in between. Dr. Ogoina tried to tell us that in 2017. We chose not to fund the system that would have let us hear him in time.

We do not neglect global health because we cannot afford to fund it. We neglect it because we have decided — through budget lines and policy choices — that some lives are worth protecting and others are worth monitoring. Until the disease stops respecting that distinction. Which it always does. Which it always will.

To every government currently proposing to eliminate funding for Gavi, to withdraw from the WHO, to zero out the Global Fund — I want to ask a simple question. When the next Dr. Ogoina sits in a clinic in a country you have defunded, watching a disease change in ways that should alarm the entire world, and tries to send that warning through the surveillance networks you have dismantled — who exactly do you think is going to hear it? And when it arrives at your border six years later, having traveled on the wings of your own indifference, what exactly is your plan?

The price of abandoning global health will be paid. It has always been paid. But this time, with gutted surveillance, defunded allies, and a public already primed to distrust the institutions designed to protect them, the cost will arrive faster, spread further, and demand more. And unlike a vaccine, regret offers no immunity whatsoever.

We put a man on the moon in 1969. We eradicated smallpox from the entire planet in 1980. In 2026, measles is back. Let that sit for a moment.

As of last week, 1,748 Americans have been diagnosed with measles in 2026 — a disease this country declared eliminated twenty-six years ago. The outbreak spans 33 states. Ninety-two percent of those infected were unvaccinated. The United States now stands at serious risk of losing its measles-free status when international health authorities conduct their review this November. It would not be the first country to lose it. It would, however, be the most powerful — and its choices are pulling the rest of the world down with it.

The resurgence did not arrive by accident. It was built — slowly, through eroding public trust, political interference in science, sweeping budget cuts, and the quiet, deeply human fear that parents like me carry into every pediatrician’s waiting room. Each of those forces matters. But they are not equal. Fear can be met with compassion and information. Political destruction is a different thing entirely.

Under Health Secretary Robert F. Kennedy Jr., the CDC reduced its recommended childhood vaccine schedule from 17 vaccines to 11 — eliminating universal recommendations for rotavirus, influenza, meningococcal disease, hepatitis A and B, RSV, and COVID-19. The changes were made without the agency’s standard expert review. The American Academy of Pediatrics broke with the CDC for the first time in its history and declared that the federal government “can no longer be trusted in this role.” Proposed federal budgets would slash the CDC by more than 50%, zero out U.S. funding for Gavi, the Vaccine Alliance, and end American participation in the World Health Organization. More than $600 million in public health grants to states have been canceled. Vaccine clinics have closed. Surveillance systems have gone dark.

The Mother in the Waiting Room

I want to be honest with you, because I think honesty is the only thing that actually helps anyone. When my daughter was born, I was terrified of vaccines. Not because I had read anything particularly alarming, not because a celebrity had scared me, but because I loved this tiny human more than I had ever loved anything — and love, when it is that new and that raw, makes you question everything.

I had been vaccinated myself. I grew up fine, no issues I am aware of. But somehow that logic, the logic of my own healthy, vaccinated body, did not fully translate when it came to my child. She was different. She was mine. And the stakes felt infinitely higher.

I took her to every appointment. I showed up every single time. But my skepticism was so deep that I would not allow anyone, not even a doctor, to hurt her. And in my mind at the time, that is exactly what a shot was: hurting her. So she fell behind on her vaccine schedule, not because we were not there, but because I would not let it happen when I still questioned the science.

I had done my due diligence by performing research. When the appointment came, I asked questions. As many as I could think of. But the answers never fully satisfied me; they felt like protocol, not conversation. A doctor moving through a checklist, not a mother being truly heard. And then when the catch-up visit finally came and my daughter received four, maybe five shots at once, her tiny body reacted. A fever. Small bumps on her skin. Watching her go through that, helpless and hurting, was one of the hardest things I have ever experienced as a mother.

I changed pediatricians. I needed someone who would slow down and actually talk with me; not hand me a pamphlet and move on. Eventually, I found one who did. And what she told me changed everything.

What the Science Actually Says

This is what my new pediatrician finally explained to me, calmly and without a checklist.

Vaccines work by training your immune system to recognize a threat before that threat becomes dangerous. They introduce a weakened or inactivated form of a pathogen and your body builds a defense. When the real thing arrives, your immune system already knows what to do. The schedule is not arbitrary — it is designed around the windows when children are most vulnerable and their immune systems most responsive. And the catch-up approach, she told me, exists for the same reason: the cost of waiting is higher than the discomfort of a difficult appointment. I did not want to hear that. But I could not argue with it.

As for the reaction my daughter had, the fever, the small bumps, those are actually signs that the immune system is doing exactly what it is supposed to do. Responding. Building. I wish someone had told me that, clearly and in advance, before I sat in that waiting room gripped by fear.

The World We Have Forgotten

I did not know any of this until I went looking. It is easy to be skeptical of something when you have never seen what it prevents. That is, in a strange way, one of vaccines' greatest successes — and greatest communication failures. We have forgotten what the world looked like before.

Polio paralyzed tens of thousands of American children every year in the early 1950s. The iron lung was a real machine that real children spent their lives inside. Measles infected nearly every child in America before a vaccine existed; most recovered, some went deaf, some developed brain damage, some died. Whooping cough could last three months and in infants could simply stop their breathing. Smallpox killed an estimated 300 million people in the 20th century alone. In 1980 it was eradicated from the entire planet — the only human disease ever fully eliminated. It happened because of vaccination. Full stop.

This Is Not Only an American Story

What happens inside the borders of the United States does not stay there. Disease does not respect passports. And when the most powerful nation on earth withdraws its funding, destabilizes its own public health infrastructure, and exports skepticism through the loudest megaphone in the world, the consequences fall hardest on the families least able to absorb them.

Globally, over 14 million infants, most of them in Africa and South Asia, did not receive a single basic vaccine in 2024. These are children born into circumstances where the barrier to immunization is not ideology or fear. It is poverty, conflict, and collapsing supply chains. For those families, a vaccine is not a debate. It is a prayer. And the funding that made those vaccines possible is being dismantled.

The WHO’s Global Measles and Rubella Laboratory Network, the surveillance system that watches for outbreaks before they become catastrophes, secured only 15% of its 2026 funding, partly due to the withdrawal of U.S. contributions. Gavi, the Vaccine Alliance, which has vaccinated more than one billion children in lower-income countries and averted an estimated 17 million deaths, has had its U.S. funding proposed for elimination entirely. Nearly half of low- and lower-middle-income countries surveyed by the WHO are now facing moderate to severe disruptions to vaccination campaigns due to reduced donor funding.

The cruelest irony is this: the countries doing the most to dismantle global immunization infrastructure are the ones whose children are protected enough to survive the consequences of that choice. The children who will die from the diseases that return are not in Washington. They are in places where a mother has no pediatrician to switch to, no second opinion to seek, no political system to hold accountable. They will simply bury their children; for decisions made in rooms they will never enter, by people who will never know their names.

What Politicians Must Do

I am a mother, not a policy analyst. But I have learned that the difference between a child being vaccinated and a child going unvaccinated often comes down to one thing: trust. Right now, politicians at every level are either building that trust or destroying it. There is very little middle ground.

I want to say something directly to every elected official, from school board members to United States senators: parents like me were not unreasonable. We were scared, and we were not given the tools to work through that fear. When public health messaging is confusing, contradictory, or politicized, hesitant parents do not trust vaccines more. They trust them less. Every mixed message from a health official, every budget cut that closes a clinic, every unfounded claim amplified by someone in power — these are not abstract policy decisions. They are invitations to doubt. And doubt, in the context of infectious disease, costs lives.

Diseases do not check your voter registration. Measles does not care about your political affiliation. The protection immunization provides belongs to everyone — and so does the obligation to protect it.

What I Would Tell My Earlier Self

My daughter is vaccinated now. All caught up. She is healthy and stubborn and wonderful and I am grateful every single day. Looking back, I do not regret being cautious. I regret not being better equipped to process my fear productively. I needed information. I needed compassion. And I needed a medical system willing to give me both at the same time. I was scared — and fear, when it is met with patience and truth, can be changed.

If you are a first-time parent who is scared right now, I see you. Your love is real. Your fear is real. And vaccines are still one of the most important gifts modern medicine can offer your child. Those two things are not in conflict.

There is a generation of parents like me, confused, cautious, doing our best in the dark, who chose to forget what these diseases could do. That forgetting was human. Understandable. It came from the very success of the system we were now questioning. We had never seen an iron lung. We had never buried a child from measles. We forgot.

But forgetting is different from what is happening now. The system that protected our children, built over decades, paid for in research and sacrifice and the quiet heroism of public health workers the world will never know, is being dismantled in public, in broad daylight, piece by piece. And some people are cheering.

That is no longer forgetting. That is choosing not to protect them. And history , the history written by the children who survive, and the parents who do not get to watch them grow up , will not be kind about the difference.

For millions of families, poverty is not merely a circumstance but an inheritance — passed down as silently and as surely as eye color. Understanding how this cycle works is the first step toward dismantling it.

Poverty leaves footprints. They appear in infant brain scans shaped by chronic stress, in the academic records of children who changed schools three times before fifth grade, in the credit scores of adults who grew up without anyone to co-sign a loan or explain what interest means. These footprints do not vanish when a child grows up. They deepen — and they are passed on.

Generational poverty, sometimes called intergenerational or cyclical poverty, describes the condition in which poverty persists across two or more generations of a family. It is distinct from situational poverty — the kind caused by a sudden job loss or medical crisis — because it is structural: baked into the institutions, neighborhoods, social networks, and psychological frameworks that shape a person’s life from birth.

To break this cycle requires more than charity. It demands a clear-eyed understanding of how the cycle works, and the courage to intervene at every link in the chain.

How the cycle sustains itself

Generational poverty is self-reinforcing because it operates across multiple dimensions simultaneously. Economic deprivation is never just about money — it is about time, health, knowledge, safety, and social capital. Each dimension compounds the others.

Children raised in poverty are more likely to attend under-resourced schools, live in neighborhoods with higher exposure to environmental toxins and violence, suffer from untreated health conditions, and experience the kind of chronic, unpredictable stress that researchers call “toxic stress” — a state that physically alters brain development, impairing executive function, memory, and emotional regulation. These effects are measurable. They are not destiny, but they stack the odds.

Access to social capital — the networks of people who can write a reference, offer a job lead, or explain how to navigate a bureaucratic system — is distributed deeply unequally. A child whose parents went to college grows up surrounded by people who know how college admissions work, how to negotiate a salary, how to build credit. A child whose parents did not lacks this informal curriculum entirely. The gap is invisible on paper, but it is vast in practice.

Housing instability compounds everything. Families who move frequently to find affordable rent accumulate none of the neighborhood roots — the trusted neighbors, the known teachers, the local job contacts — that form the connective tissue of economic resilience. And because American school quality is tied to property taxes, living in a poor zip code is, in itself, an educational disadvantage.

The role of structural barriers

Individual effort matters enormously. But effort alone has never been sufficient to overcome structural barriers, and pretending otherwise is both dishonest and cruel. Redlining, discriminatory lending, the gutting of union labor protections, mass incarceration, and the chronic underfunding of public schools in low-income areas are not ancient history. Their effects are measurable in neighborhood income data today.

The racial dimensions of generational poverty in the United States are stark. The median white family holds roughly eight times the wealth of the median Black family — a gap traced directly to decades of legal exclusion from wealth-building programs like the GI Bill, FHA loans, and Social Security in its original form. Wealth, unlike income, compounds across generations: it funds down payments, pays for college without debt, provides a safety net during hard times. Families excluded from building it start each generation behind.

Factors that perpetuate the cycle

• Underfunded schools in low-income zip codes

• Limited access to healthcare and mental health services

• Housing instability and lack of homeownership

• Absence of inherited wealth or financial safety nets

• Exposure to toxic stress and adverse childhood experiences (ACEs)

• Social capital gaps — limited access to professional networks

• Criminal justice involvement creating lasting employment barriers

What actually breaks the cycle

Decades of research and real-world intervention have produced a reasonably clear picture of what works. The findings cut across partisan lines — they are empirical, not ideological.

Early childhood investment produces the highest returns of any anti-poverty intervention. Nobel laureate economist James Heckman has documented that every dollar invested in high-quality early childhood education for disadvantaged children yields seven to thirteen dollars in long-term returns — through higher graduation rates, greater lifetime earnings, reduced incarceration, and lower healthcare costs. The window is early, and it closes.

Direct cash transfers — unconditional income support to families — consistently outperform skeptics’ predictions. Studies of programs like the Child Tax Credit expansion, earned income tax credits, and international conditional cash transfer programs show that families use money wisely. Child poverty rates fall. School performance improves. Health outcomes improve. The assumption that poor families mismanage money is not supported by evidence.

Housing stability is a prerequisite for almost every other intervention. Children who move frequently cannot form the stable relationships with teachers and peers that support learning. Section 8 voucher studies show that families moved into low-poverty neighborhoods see significant improvements in children’s long-term outcomes — but voucher supply is catastrophically inadequate relative to need.

Access to quality healthcare and mental health support — including treatment for the trauma that poverty inflicts — breaks feedback loops that would otherwise persist. Untreated depression, anxiety, and PTSD in parents impair their ability to provide the stable, warm caregiving environments that children need.

College and vocational training access, stripped of the debt loads that now burden students from low-income families, remains one of the most reliable pathways out of poverty that exist. First-generation college students navigate a labyrinth without maps; mentorship, advising, and financial support dramatically change completion rates.

The community dimension

No policy intervention works in isolation, and none works without community. The most durable change emerges when low-income communities have genuine agency over the institutions that serve them — when residents help design programs, when local organizations are funded rather than bypassed, when the people closest to a problem are trusted to shape its solutions.

Mentorship matters. The presence of even one stable, caring adult in a child’s life — a teacher, a coach, an aunt, a neighbor — is among the most powerful predictors of resilience in the face of adversity. This cannot be programmed at scale, but it can be cultivated: in churches, in community centers, in schools that are resourced enough to let teachers actually know their students.

Social entrepreneurship and local economic development — businesses owned by community members, rooted in the neighborhood, reinvesting locally — build wealth in ways that national employers and absentee landlords do not. Worker cooperatives, community land trusts, and credit unions all serve as mechanisms for keeping capital circulating within communities that have historically seen it extracted.

A problem of will, not knowledge

We largely know what works. The evidence base for breaking the cycle of generational poverty is not weak. What has been lacking — in every country that struggles with persistent poverty — is the sustained political will to implement solutions at sufficient scale and over sufficient time.

Anti-poverty programs are routinely cut before they have time to demonstrate results. Political cycles are short; childhood development is long. The people who benefit most from effective programs — children — cannot vote. The people who fund political campaigns rarely grew up poor. These are structural features of democracy, not bugs, but they create systematic biases against the long-term investments that poverty elimination requires.

That is why public awareness matters. Every person who understands how generational poverty actually works — who can see the feedback loops, who knows what the evidence says — makes it marginally harder to dismiss the children at the bottom of the distribution as simply unlucky, or as the inevitable casualties of a system working as designed.

The cycle is not inevitable. It has been broken — by individuals through extraordinary effort, and by societies through deliberate policy. Knowing that it can be broken is where every serious effort to end it must begin.

She has brought her pay stubs, her lease, her utility bill, her children’s birth certificates, and a note from her landlord. She is applying for food assistance. She has been hungry for three days. The form asks her to explain, in writing, why she needs help.

This is not a bureaucratic accident. It is policy. The United States has built one of the most elaborate systems of means-testing, work requirements, and eligibility verification in the developed world — a vast machinery designed not primarily to help the poor, but to decide which poor people are sufficiently deserving to receive help at all.

The philosophy underlying this system is rarely stated plainly. But its logic is everywhere: public assistance is not a right but a privilege, and privileges must be earned. Poverty itself is insufficient evidence of need. You must also prove your poverty was not your fault, that you are working to escape it, and that you are not somehow gaming a system that has, in nearly every measurable way, already won.

The United States has never fully accepted that poverty is a structural condition rather than a personal failing. That ambivalence is encoded into law.

The Paperwork as Punishment

Consider the Supplemental Nutrition Assistance Program — SNAP, formerly called food stamps. To qualify, a family must submit documentation of income, assets, household composition, and residency. They must recertify, often every six months. In many states, they must attend in-person interviews. Miss an appointment, fail to return a form within a narrow window, or submit documents with a minor error, and benefits are cut — not temporarily delayed, but terminated, requiring the entire application process to restart.

A 2021 study found that among eligible families who do not receive SNAP, the most common reason is not disqualifying income but administrative burden: the time, documentation, transportation, and comprehension required to navigate enrollment. For someone working two jobs, caring for children, and managing the specific chaos of poverty — unpredictable income, unreliable transportation, frequent moves — those administrative requirements function as a wall.

This is not a failure of the system. It is, for many of its architects and defenders, a success. The administrative burden serves a function: it rations access. It ensures that only the most motivated, organized, and — frankly — least desperate people successfully obtain benefits. The truly overwhelmed fall away. The paperwork is, in this sense, doing its job.

The Myth of the Welfare Cheat

Means-testing is the fear of fraud — the specter of the undeserving poor, the “welfare queen,” the person who could work but chooses not to, draining public resources out of laziness or cunning. This figure has haunted American political discourse since at least the 1970s, wielded with particular effectiveness to build opposition to programs that primarily serve Black Americans.

The empirical reality is almost comically different. Benefit fraud in federal assistance programs runs at roughly 1 to 4 percent — comparable to fraud rates in Medicare, tax deductions for charitable giving, and the small business loans distributed in the early months of the COVID-19 pandemic, none of which generated political movements to make recipients prove their worthiness through quarterly paperwork submissions.

The disproportion is instructive. When wealthy Americans shelter income in offshore accounts, that is tax planning. When corporations claim credits they may not fully deserve, that is routine accounting. When a poor family fails to report a $200 birthday gift from a relative and thereby temporarily exceeds SNAP’s asset limit, that is fraud — and it can result in permanent disqualification from the program.

We have created a society where the scrutiny applied to a poor person seeking $200 in food assistance is incomparably greater than that applied to a corporation seeking millions in tax relief.

Work Requirements: The Purest Expression

No mechanism more cleanly expresses the morality underlying American welfare policy than the work requirement. The logic is elegant in its cruelty: if you are not working, you do not deserve to eat. Or rather, you may eat — but only if you can document, to the government’s satisfaction, that you are trying hard enough to stop needing to eat for free.

Work requirements have been a feature of TANF (Temporary Assistance for Needy Families) since welfare reform in 1996. They have periodically attached themselves to Medicaid, SNAP, and public housing. They were proposed for, and briefly applied to, pandemic emergency assistance. Their political appeal is durable because they activate a powerful moral intuition: reciprocity. Help should be earned. Assistance should come with obligation.

What they actually do, in practice, is studied and consistent. They reduce enrollment. They do not, with any reliability, increase employment. A sweeping 2021 review of work requirement literature found that Arkansas’s Medicaid work requirement, implemented in 2018, caused 18,000 people to lose health coverage in ten months. The vast majority were not unemployed — they were employed but unaware of the reporting requirements, or unable to submit documentation in time, or employed in jobs the system did not count as qualifying work: informal labor, caregiving, seasonal employment.

This is the pattern everywhere work requirements are studied. They do not move people from welfare to work. They move people from welfare to nothing — to the condition of being poor without assistance, which is simply being poor with fewer resources. The stated goal and the achieved outcome are not merely unaligned; they are opposite.

The Surveillance State for the Poor

What few commentators note is how deeply the administration of poverty in America resembles surveillance. SNAP recipients are subject to electronic benefit transfer (EBT) tracking that logs every purchase. Some states have proposed, and a few have implemented, restrictions on what can be purchased: no seafood, no steak, no birthday cake. The message is not subtle. You are not trusted with money. You are not trusted with choice. You are not even trusted to celebrate your child’s birthday with the food you personally select.

Drug testing for welfare recipients — despite being repeatedly struck down or found ineffective — resurfaces in state legislatures every few years. Tennessee, Missouri, and a dozen other states have implemented or attempted such programs. Studies consistently show they are expensive, catch almost no one, and function primarily as a ritual of humiliation: a performance of suspicion that reminds recipients they are being watched, judged, and presumed guilty until proven otherwise.

Meanwhile, the wealthy receive their own extensive system of government transfers — mortgage interest deductions, capital gains preferences, 529 college savings accounts, inheritance structures — with no means-testing, no work requirements, no drug tests, and no requirement to document how they spend the money. The double standard is so complete as to be nearly invisible.

The Cost of Cruelty

There is, of course, an economic argument for all of this. Means-testing, its defenders say, ensures that limited resources reach those who need them most. Work requirements encourage self-sufficiency. Verification reduces waste. The administrative complexity is the price of fiscal responsibility.

The actual economics tell a different story. A comprehensive 2019 analysis found that the administrative cost of means-testing SNAP — processing applications, conducting interviews, verifying documents, handling appeals from incorrectly denied applicants — consumed roughly 22 cents of every dollar spent on the program. A universal program without means-testing would cost more in total, but significantly less per person helped, because the bureaucratic overhead of deciding who doesn’t qualify would vanish.

This arithmetic is well understood among policy researchers. It is largely ignored in policy debates, because the point of means-testing was never primarily efficiency. It was always, at least in part, something else: the satisfaction of ensuring that assistance came with moral judgment attached. That the poor were sorted into deserving and undeserving. That suffering alone was not sufficient claim on public resources.

The philosopher Elizabeth Anderson calls this “expressive harm” — the damage done not by the material deprivation of a policy, but by what it communicates. When the government requires a hungry person to prove their hunger before they may eat, it is expressing something about how it regards that person. It is saying: your need is suspected. Your honesty is doubted. Your poverty requires explanation.

What Other Countries Do

The United States is not without alternatives. Most wealthy democracies — Germany, Canada, the Nordic countries, the United Kingdom — provide some combination of child allowances, housing subsidies, and health coverage that are either universal or require far less burdensome verification. Germany’s child benefit (Kindergeld) is available to all families with children regardless of income, with adjustments made through the tax system rather than the welfare system. Canada’s Child Benefit provides monthly payments with income-based reduction built in, but without the documentation theater American recipients must perform.

The results are measurable. Child poverty rates in peer nations are substantially lower than in the United States. This is not because those countries are richer — many have lower GDP per capita. It is because their programs actually reach children. Universality, it turns out, is effective precisely because it does not require proving you deserve what you need.

The American response to this evidence is generally ideological: those countries are different, their populations more homogeneous, their cultures more collectivist, their governments more trusted. What is almost never acknowledged is that the American resistance to universal programs is itself a choice — a continuous political decision to prefer a system that humiliates some over a system that might benefit all.

The Human Cost, Measured Carefully

Behind every statistic in this argument is a person in a waiting room. Social science has tried to measure what happens to people who are poor in America’s particular way — not just economically poor, but subject to this specific ecosystem of verification, surveillance, conditionality, and moral audit.

• Children who experience churn in SNAP enrollment — periods of benefit interruption due to administrative issues rather than changed eligibility — show measurable increases in food insecurity, emergency room visits, and, in longitudinal studies, reduced educational attainment.

• Adults who interact frequently with the benefits system show elevated stress biomarkers consistent with chronic psychological threat response — not from poverty alone, but from the specific experience of conditionality and potential termination.

• Medicaid enrollees subject to lock-out periods or documentation requirements delay or forgo preventive care at significantly higher rates than enrollees with stable, unconditional coverage — with corresponding downstream health costs that exceed the savings from deterring enrollment.

• Housing assistance applicants who must navigate multi-year waitlists, income verifications, and behavioral requirements report psychological effects — anxiety, shame, the internalization of worthlessness — that researchers describe as distinct from and compounding the effects of housing instability itself.

The architecture of American poverty assistance does not merely fail to solve poverty. It actively produces harm — measurable, documented harm — as a byproduct of its insistence on moral sorting. The question this research invites is not whether we can afford to do better. It is whether we have decided not to.

A System Working as Designed

The hardest thing to accept about American welfare policy is that most of it is not broken. It functions exactly as intended. The churn, the administrative burden, the documentation requirements, the work rules — these produce the outcomes they were designed to produce: lower enrollment, lower costs, and the preservation of a moral narrative in which poverty is a personal condition rather than a structural one.

This narrative matters because it licenses inaction. If the poor are poor because of choices, character, or failure to comply with reasonable requirements, then the scale of American poverty is not a policy problem requiring a policy solution. It is a natural distribution of outcomes in a meritocratic system — unfortunate but not unjust, and certainly not the responsibility of a government that already offers help to those who truly deserve and earnestly seek it.

The reality is that 37 million Americans live in poverty with the majority of them being women, children, elderly, and/or disabled. Millions more live in precarity — one medical bill, one job loss, one car repair away from it. The systems designed to catch them when they fall are riddled with holes, laced with barbs, and ringed with paperwork. They catch some people. They wound many of the people they catch. And they turn away, quietly, efficiently, and with the full authority of bureaucratic procedure, millions of people who are eligible, who need help, and who will not get it.

Not because the money ran out.

Because they couldn’t prove they deserved it.

The United States spends enormous political and administrative energy determining who is sufficiently poor, sufficiently virtuous, and sufficiently persistent to receive public help. Other nations spend that energy simply helping people. The difference in outcomes is not a mystery. It is a choice — made annually in legislatures, encoded in regulations, and enforced, form by form, in waiting rooms across the country.

As a municipal government employee that provides housing services, I am all for America First policies that help families, the disabled and the homeless. Funding homeless and housing services is one of the many ways our federal government puts Americans in the forefront of what many would call “American First” policies. I am referring to policies that provide federal rental assistance to 10.1 million American, of which 69% are seniors, children and individuals with disabilities. The kind of policies that should center around uplifting American families that are struggling at a time in which the rental cost is at an all-time high.

This is why it is concerning to see that the president FY27 budget request includes significant cuts toward rental assistance and homelessness services. The president and HUD Secretary Scott Turner have suggested a $3.8 billion cut towards housing, homelessness and community development programs. These cuts could halt housing authorities from providing new vouchers to families that have not previously received federal housing assistance and do not consider the rising cost of living that seems to increase with every passing year. However, it is not just housing that is being affected. Other “America First” programs that would be affected include the elimination of Job Corps, $4 million cuts towards Low Income Home Energy Assistance Programs and over $8.5 million cuts towards more than two dozen K12 grant programs.

The rationale behind these cuts is to increase defense spending. The President Trump’s FY27 budget request seeks to increase defense expending by $1.5 trillion, which would be a 42% increase from FY26. The increase in defense spending would be to fund a conflict that many Americans do not agree with. According to a recent poll by the Pew Research Center found that 59% of Americans reported that the United States made the wrong decision in using military force against Iran. Now it is no secret that funding these kinds of conflicts increases the national debt. Now the current administration is trying to take away funding from programs that are critical to many Americans. This is why I call on my fellow Americans to consider voting for their best interests in the coming midterm elections.

America First means choosing representatives that have our best interest. Funding a conflict on the belief that we are being good allies only takes away from everyday Americans and risks the lives of our military. Many would argue that supporting policies that fund housing, education and community development create an illusion of a welfare state. Others have labeled me a socialist for advocating for such programs in the past. That is perfectly fine. I can be labeled as a socialist, but I am also a patriot. Like my fellow RESULTS advocate, Tiffany Tagbo, I am also of the belief that cutting funding towards social services program only increases the need for them. The economically disfranchised are always carrying the burden and it is time to turn the page.

These are not just talking points to me. After all, I am an American that is about America First.

There is a pattern in how powerful institutions respond to poverty. They do not eliminate it. They make it more expensive, for everyone except the people whose decisions created it in the first place. They cut the programs that prevent crisis, then spend far more managing the crises that follow. They call this fiscal responsibility. History tends to call it something else.

In 2024, the U.S. Census Bureau counted 35.9 million people living in poverty, with an official poverty rate of 10.6 percent. USDA data showed that 13.7 percent of American households were food insecure that year, including nearly one in five households with children. These are not the numbers of a country without resources. They are the numbers of a country that has repeatedly chosen not to use those resources to prevent hunger.

That choice carries costs that never appear in the budget proposals used to justify it. The Bureau of Justice Statistics reports 1.85 million people incarcerated in U.S. prisons and jails, with another 3.7 million on probation or parole. Incarceration costs more per person each year than many American universities. Emergency room visits cost more than preventive care. Homeless shelters cost more than housing assistance. At every turn, the cheaper intervention is the one cut first. What replaces it is more expensive, more traumatic, and less effective, and the people who made the cut are rarely the ones forced to absorb the consequences.

This is not just cruel policy. It is shortsighted policy.

History offers no shortage of warnings. Roman rulers understood that a city full of hungry people was a city that could not be governed, and they built the grain dole accordingly. Not out of compassion, but out of self-interest. Whatever finally brought Rome low, it was not an excess of generosity toward the poor.

France offers the sharper lesson. By January 1789, the price of bread in Paris had more than doubled. That fact alone did not cause the French Revolution. But it made everything else combustible. It turned suffering that had been private and scattered into something collective and explosive. The Bourbon monarchy had centuries of power and chose, at the margins, not to feed its people. Within months, it had neither power nor choice.

Russia in 1917 followed a similar script. The February Revolution did not begin with a manifesto. It began in the bread queues of Petrograd, where ordinary people, mostly women, had run out of patience with being told to survive on nothing. What began as hunger became, within days, the collapse of a dynasty. Nicholas II did not fall because a superior political force outmaneuvered him. He fell because enough people had nothing left to lose.

None of this means that cutting SNAP benefits will produce a revolution. The point is narrower, and more uncomfortable. Governments that treat the suffering of poor people as a line-item problem tend to discover, too late, that desperation does not stay contained. It does not remain neatly within the neighborhoods where it is manufactured. It moves. It shows up in emergency rooms, in classrooms, in shelters, in jails, in overworked public systems, and in homes where children learn far too early what instability feels like. It accumulates quietly until it becomes too large, too visible, and too expensive to ignore.

The current push to cut the federal safety net is not happening because the United States cannot afford to feed, house, or care for its most vulnerable people. It is happening because the people making these decisions have calculated, correctly for now, that the damage will land somewhere else. On families rationing insulin. On emergency departments absorbing uncompensated care. On communities cycling through homelessness, incarceration, untreated illness, and early death.

And now, increasingly, they are trying to make that damage harder to measure. The USDA has indicated that the 2024 Household Food Security report, the source for the figures cited here, will be its last. A government that cuts aid and then eliminates the data that measures the harm is not practicing fiscal responsibility. It is practicing concealment.

History rarely rewards governments that deepen desperation among the poor and assume the consequences will stay contained. More often, it records the opposite, in different languages, in different centuries, with the same basic result. When leaders treat human suffering as manageable collateral, they eventually learn that it is neither manageable nor collateral.

Cutting benefits is a choice. So is what comes after.

Lawmakers must reverse these cuts, to SNAP, to Medicaid, to housing assistance, and to every program that stands between a family and a crisis they cannot survive alone. Not scale them back more slowly. Not replace them with something weaker. Reverse them.

A government can hide suffering in spreadsheets for a while. It can move it offstage, bury it in emergency rooms, shelters, jails, and obituaries. But it cannot make that suffering disappear. Sooner or later, the bill comes due. The only question is who will be forced to pay it.

Share Emerging Leaders of America

Every April 11 through 17, Black Maternal Health Week (BMHW) draws national attention to one of the most documented and most neglected public health crises in the United States. Founded in 2018 by the Black Mamas Matter Alliance (BMMA), the campaign has grown from a grassroots awareness effort into a powerful policy platform. Now in its tenth year, the 2026 theme “Rooted in Justice & Joy” signals both the movement’s maturity and the urgency of the work still undone.

But before policymakers and advocates can act with real urgency, they must understand where this crisis comes from. Because it did not begin with a data point. It began with a history: one that still walks into every delivery room today.

A Crisis Built Over Centuries

The modern Black maternal health crisis cannot be separated from American medicine’s foundational relationship with Black women’s bodies, one defined by exploitation without consent.

James Marion Sims, often called the “father of modern gynecology,” advanced his career by experimenting on enslaved women in the South without anesthesia, justified by the false and persistent myth that Black people experience pain differently than white people. The three most documented of his subjects, Anarcha, Betsey, and Lucy, were subjected to repeated procedures yet received no credit, no compensation, and for more than a century, no recognition. They were among roughly a dozen enslaved women Sims operated on, and he trained them to assist with the procedures, effectively making them surgical nurses without granting them any of the status or acknowledgment that he claimed for himself.

This was not an isolated aberration. In the 1920s, eugenics became widely accepted by a segregated medical industry, and states passed sterilization laws targeting Black women and others deemed “undesirable.” From 1932 to 1972, the U.S. Public Health Service conducted the Tuskegee Syphilis Study, withholding treatment from nearly 400 Black men with syphilis even after penicillin became the standard cure, solely to observe the disease’s progression. Black women were largely excluded from the study’s formal documentation, yet their health and the health of their children were directly affected, as syphilis can be passed from mother to baby during birth.

This history is not merely a moral indictment. It is a clinical explanation. The same attitudes that enabled experimentation on Black bodies without consent continue to shape the way Black women are treated in healthcare settings today, including the documented tendency for providers to dismiss their symptoms and underestimate their pain. The distrust many Black women feel toward the medical system is not irrational hesitancy. It is a learned, rational response to institutions that have repeatedly harmed them. Policymakers who want to improve Black maternal outcomes must begin by acknowledging this history openly and building systems that earn trust rather than demand it.

The Numbers Are Still Unacceptable

Against this backdrop, the contemporary data lands differently.

The CDC’s 2024 data show that Black women in the United States died of maternal causes at a rate of 44.8 deaths per 100,000 live births CDC, more than three times the rate for white women (14.2). In 2023, that gap had widened even as overall maternal mortality declined: white women saw their rate fall significantly to 14.5 per 100,000, while the rate for Black women held at over 50, more than three times higher and significantly above Hispanic women at 12.4 and Asian women at 10.7. Policycentermmh Black women are not just doing worse than white women. They are dying at higher rates than every other racial and ethnic group tracked by the CDC, and the disparity is growing wider, not narrower.

A McKinsey Health Institute analysis projects that if current trends continue, the Black maternal mortality rate could nearly double to 94 deaths per 100,000 live births by 2040, placing the United States on par with many low-income countries. McKinsey & Company For a nation that spends more per capita on healthcare than any other, this is not a projection about the future. It is an indictment of the present.

Where the Policy Gaps Are Deepest

Understanding the crisis means understanding where the system fails Black mothers most clearly.

Medicaid coverage gaps remain life-threatening. Postpartum Medicaid coverage has been extended to 12 months in many states, but not all. States that have not expanded Medicaid under the Affordable Care Act leave Black mothers disproportionately exposed. Affordable Care Act and Medicaid funding have been reduced by more than $1 trillion under the current administration and congressional Republicans, affecting programs that cover over 40 percent of births. These changes have coincided with more than 40 maternity care providers closing services, including labor and delivery units. Onceuponahill Advocates must resist further cuts and push for full Medicaid expansion in every remaining state.

Maternal care deserts are concentrated in Black communities. Many Black women live in regions with few or no obstetric providers, particularly in states that have resisted Medicaid expansion, and the fragmentation of postpartum healthcare coverage further compounds these risks. nih Incentivizing rural and underserved practice through loan forgiveness, expanding telehealth infrastructure, and investing in community birth centers are all evidence-based interventions that can reach women where they are.

Data infrastructure remains incomplete. More than 80 percent of pregnancy-related deaths in the United States are preventable IWPR, according to maternal mortality review committees, but those committees are only as useful as their funding allows. Federal investment in MMRCs, and in data systems that disaggregate outcomes by race and ethnicity in real time, is not optional. It is foundational.

Implicit bias in clinical settings is a systems problem, not just a training problem. One of the most persistent policy mistakes is framing bias as an individual awareness issue rather than a structural one. That approach leads to workshops instead of reform. But bias in maternal care is often built into the tools and protocols that guide care itself. Black women are more likely to undergo cesarean delivery for subjective fetal heart rate interpretations, and race-based correction factors in anemia care may contribute to undertreatment and poorer outcomes. These are not simply attitudinal failures. They are embedded in clinical algorithms, decision-making tools, and institutional defaults.

That is why real progress comes from changing systems, not just asking individuals to reflect. California hospitals reduced Black-white disparities in severe maternal morbidity through a hemorrhage quality improvement collaborative, and standardized labor induction protocols narrowed disparities in cesarean deliveries and neonatal morbidity. Cedars-Sinai increased aspirin recommendations among Black patients for preeclampsia prevention simply by changing a clinical default, raising the rate from 23.6 percent to 58.1 percent and effectively eliminating the racial disparity in aspirin administration. These outcomes came from redesigning care, not merely discussing bias.

At the legislative level, California has required continuing education tied specifically to implicit bias and Black maternal care through laws AB 241 and SB 464, while Maryland has implemented its own mandatory training tied to licensure renewal. Other states should follow, but training alone is not enough. Accountability must go further. Hospitals should publicly report maternal outcomes disaggregated by race on a continuous basis. Patients should have a documented pathway to report discriminatory or dismissive treatment. Accreditation bodies should build racial equity into hospital standards. And race should be removed from clinical algorithms wherever it is being used as a biological proxy instead of being understood as a marker of exposure to systemic racism and unequal care.

The Evidence for Community-Based Solutions

For Black mothers, support, visibility, and collective care are not extras. They are essential. That truth is not just emotional. It is measurable. Research shows that doula support is associated with fewer unnecessary cesarean sections, fewer preterm births, and more positive birth experiences overall. A major multi-site study found that women who received doula care had substantially lower odds of cesarean delivery and postpartum depression or anxiety.

City-level data backs that up. An audit of New York City’s Citywide Doula Initiative found that Black and Hispanic participants experienced lower rates of cesarean births, preterm births, and low birth weight babies compared with the general city population. The program was designed to address structural racism and socioeconomic barriers, and it produced measurable results.

This matters especially because Medicaid covers the majority of births among Black and American Indian or Alaska Native individuals, yet doula services remain only partially covered or entirely uncovered in many state Medicaid programs. Expanding Medicaid reimbursement for doulas and midwives is one of the highest-impact policy actions available. It costs less than managing preventable complications and does more to center care around the patient rather than the institution.

There is also history here. Before birth was moved into hospitals, Black “granny midwives” played a central role in labor and delivery, especially during and after enslavement. As birth became increasingly medicalized, midwifery was marginalized and dismissed while labor and delivery were moved into segregated hospitals controlled by white male physicians. Rebuilding midwifery and doula infrastructure in Black communities is not a trendy innovation. It is a reclamation of models of care that were more community-rooted, more relational, and often more responsive to Black women’s needs.

The Legislative Landscape: Progress and Peril

The Black Maternal Health Momnibus Act remains the most ambitious federal framework for addressing this crisis. The package addresses social determinants of health, maternal mental health, workforce diversity, data collection, support for community-based organizations, and care for incarcerated mothers. Its strength lies in its recognition that no single intervention can solve a crisis that no single cause created.

Parts of the broader Momnibus agenda have secured funding through the appropriations process since 2023, but the full framework has not yet been reintroduced in the current Congress. That makes state-level action even more important. Legislators in states including North Carolina, Michigan, Oregon, and Pennsylvania have introduced their own Momnibus-style proposals focused on issues ranging from implicit bias training to Medicaid midwifery coverage and grants for community-based maternal health organizations. That state-level momentum matters. It is where advocates should be concentrating sustained pressure right now. Victories at the state level help build the evidence, infrastructure, and political will for broader federal action.

For lawmakers outside the United States, the call to action is equally clear. Require maternal health data to be disaggregated by race and ethnicity in national reporting. Fund community-based doula and midwifery models in Black and Afro-descendant communities. Remove race as a biological variable from clinical algorithms and replace it with an understanding of racism as a social determinant of health. Support the WHO’s push to strengthen maternal health surveillance globally and advocate within your own institutions for Black maternal mortality to be treated as a human rights issue, not a statistical footnote. The United Kingdom’s national investigation into NHS maternity services and Brazil’s growing maternal health equity movement show that this conversation is happening across borders. Every legislature, health ministry, and international body has a role to play.

The Global Stakes

The pattern of Black women dying at disproportionate rates in childbirth is not uniquely American. It is a global signature of structural racism. It appears in countries with very different healthcare systems, which makes it impossible to explain away solely as a matter of insurance coverage or national spending.

In the United Kingdom, Black women are still significantly more likely to die during pregnancy or in the weeks after birth than their white counterparts, despite universal healthcare through the NHS. The UK government itself has acknowledged that progress has been too slow and that many Black women continue to report not being listened to, lacking culturally competent care, and experiencing racism in maternity services.

In Brazil, maternal mortality among women of African descent is approximately five times higher than among white women. Meanwhile, the overall U.S. maternal mortality rate already exceeds that of Canada at 8.4 deaths per 100,000 and France at 7.6. Across countries, the pattern is clear: national averages can obscure racialized disparities that remain lethal for Black women.

That has major implications for global health accountability. The WHO’s Sustainable Development Goal 3 calls for reducing the global maternal mortality ratio to fewer than 70 deaths per 100,000 live births by 2030. But national-level targets can allow countries to claim progress while Black women within those same countries continue to die at vastly disproportionate rates. International health bodies, bilateral funders, and multilateral institutions should require maternal health data to be disaggregated by race and ethnicity as a core reporting standard, not an optional supplement. A target that Black women cannot see themselves in is not a target that will save their lives.

The UK’s MBRRACE model offers one possible template, combining race-disaggregated maternal death reporting with specific recommendations for prevention. That kind of surveillance infrastructure, grounded not just in counting deaths but in learning from them, needs to be funded and replicated globally.

One Week Is Not Enough

Seven days of awareness cannot undo centuries of structural harm. Black Maternal Health Week is a vital catalyst for legislation, coalition-building, and centering voices the system has long ignored. But the crisis it names does not pause when the week ends.

The United States still has one of the highest maternal mortality rates among high-income nations. Black women remain disproportionately at risk even after controlling for income and education. And as the evidence from the United Kingdom, Brazil, Canada, and France makes clear, this is not just an American emergency. It is a global one wearing national clothing.

One week of awareness is not enough to move institutions that have tolerated this crisis for generations. What is needed is sustained policy action, durable investment, and a permanent seat at the table for Black maternal health advocates in national and global health spaces alike, from state legislatures to Congress, from hospital boards to the World Health Organization.

Black women have been carrying this work for generations. The question is whether institutions, national and international, will finally carry it with them.

Today is Tax Day — but for millions of American families, it is also something more specific: a reckoning. Today is when they learn whether the government that taxes their wages actually sees them. For a single mother working a home health aide job, a family of four renting an apartment in a city they can barely afford, or a child born into poverty through no fault of their own, the tax code is not an abstraction. It is the difference between groceries and hunger, between stability and eviction.

The Child Tax Credit, the Earned Income Tax Credit, and proposed measures like a federal Renters Tax Credit are not giveaways. They are investments — policy tools with proven track records of lifting American families out of poverty. But lawmakers in Washington have repeatedly chosen to protect and expand the wealth of the already-wealthy while leaving the most vulnerable Americans behind. The numbers are damning, and the record is clear.

The Child Tax Credit: A proven weapon against child poverty

The data does not lie. When policymakers actually commit to the Child Tax Credit — fully, for all children — it works. During the pandemic-era expansion under the American Rescue Plan Act in 2021, child poverty fell by a staggering 46 percent, dropping from 9.7 percent to just 5.2 percent, the lowest level ever recorded. Black child poverty fell by 8.8 percentage points in a single year. Hispanic child poverty fell by 6.3 points in that same year. These were not accidents of circumstance. They were the direct result of a policy choice that said every child deserves support regardless of how much their parents earn.

That expansion expired. Child poverty spiked by 5 million in 2022. And Congress — the same Congress that found trillions of dollars for tax cuts aimed at the wealthy — has consistently failed to restore what was proven to work.

Today, the credit is worth up to $2,200 per child — a modest increase from $2,000 under the recently signed One Big Beautiful Bill Act (OBBBA). But the structural problem remains untouched. The refundable portion of the credit, which is the only part that helps low-income families, is capped at $1,700 per child and subject to earnings restrictions that systematically exclude the poorest households. The result: the richest 20 percent of Americans received the largest benefit from the OBBBA’s credit changes, while the poorest 20 percent received nothing.

One in four children under 17 will receive less than the full credit or no credit at all in 2026. Half of all Black children and more than one in three Latino children are among those left behind — a statistical truth that cannot be separated from a long history of discrimination and wage suppression in low-paying work.

A tale of two tax bills: Billions for billionaires, crumbs for children

While working families scrape for every dollar of refundable credit, the One Big Beautiful Bill Act delivered extraordinary wealth to those who needed it least. The nonpartisan Congressional Budget Office estimates the law will cut taxes for the richest 10 percent of Americans by more than $14,700 per year per household. For the richest 1 percent, the annual tax cut exceeds $50,000 per household. Over ten years, the wealthiest 1 percent are slated to pay at least a trillion dollars less in taxes.

The OBBBA’s tax provisions cost approximately $4.5 trillion over the next decade. Four corporations — Amazon, Alphabet, Meta, and Tesla — whose chief executives stood beside the president at his inauguration collectively received $51 billion in federal tax breaks in 2025 alone, paying an effective tax rate of just 4.9 percent. At least 88 of the largest corporations in America paid zero federal income tax for 2025. Nvidia, which reported U.S. pretax income of $123 billion — among the greatest earnings growth in corporate history — avoided $6.8 billion in federal income taxes.

The estate tax exemption was raised to $15 million per person, permanently. The pass-through business deduction — 87 percent of which benefits the top 10 percent of earners — was made permanent. Capital gains protections were expanded for wealthy investors. Foreign investors in U.S. corporations will save $32 billion in taxes in 2026 — more than twice what the bottom 20 percent of American families will receive.

Let that sink in. Foreign shareholders of American companies got a better deal from this Congress than America’s poorest children.

The renters left out of the American Dream

The injustice of the tax code does not stop with child credits. It runs through every aspect of how Washington taxes housing — or rather, who it protects.

The federal government spends hundreds of billions of dollars subsidizing homeownership: the mortgage interest deduction, the capital gains exclusion on home sales, and property tax deductions overwhelmingly benefit higher-income households. Renters — who make up one in three American households, and who are disproportionately Black, Latino, young, and lower-income — receive almost nothing in comparable federal tax relief.

A federal Renters Tax Credit has been proposed by housing policy experts for years. The model is straightforward: authorize states to allocate federal tax credits so that assisted families pay no more than 30 percent of their income on rent. At a modest $5 billion cap, such a program could help 1.2 million families afford housing, reduce average rents by $400 per month, and lift four out of five of the poorest assisted families out of deep poverty.

Several states — Minnesota, Colorado, California — have taken up their own versions of renters’ credits, recognizing what the federal government refuses to acknowledge: that housing cost burden is one of the most powerful drivers of poverty in America, and that tax policy can address it. Minnesota’s proposed expansion would lift income eligibility thresholds and increase maximum credits to achieve parity with existing homeowner relief. These are reasonable, tested ideas. They are not radical.

The moral test of a tax code

A tax code is a moral document. It reflects what a society values and who its government is willing to fight for. Every April 15, working families file their returns and find out whose side the government is on.

The evidence assembled here is not partisan. It comes from the Congressional Budget Office, the Institute on Taxation and Economic Policy, the Center on Budget and Policy Priorities, Brookings, and the U.S. Census Bureau. These are not activist organizations. They are the institutions we rely on to tell us the truth about what our laws do to real people.

What the numbers show is this: Congress has chosen, repeatedly and with full knowledge of the consequences, to give more to those who have the most and less to those who have the least. They have watched child poverty fall dramatically when the full Child Tax Credit was enacted, and then let it expire. They have heard the arguments for a Renters Tax Credit and looked the other way. They have passed, with little hesitation, $4.5 trillion in tax relief weighted toward the wealthy — and called it beautiful.

Children are not born into poverty because they lack ambition or because their parents made bad choices. They are born into poverty because the systems around them — including the tax code — are designed to keep wealth concentrated and scarcity dispersed. Tax credits like the Child Tax Credit and EITC are among the most efficient, evidence-based, politically achievable tools we have to interrupt that cycle. Their opponents in Congress do not lack the evidence. They lack the will.

On this Tax Credit Day, as millions of families wait on refunds that will cover a month of groceries or a car repair or a child’s medicine, the contrast could not be sharper. While the wealthiest Americans count windfalls measured in tens of thousands of dollars, a home health aide raising two children on $20,000 a year gets a child tax credit worth less than half what a family earning ten times as much receives.

That is not a flaw in the system. It is the system. And the lawmakers who designed it, voted for it, and defended it deserve to be held accountable for every child it leaves behind.

When lawmakers talk about cutting benefits, they usually hide behind language like fiscal responsibility, efficiency, and reform. But there is nothing responsible about balancing a budget on the backs of poor families. President Donald Trump’s fiscal year 2027 budget makes that painfully clear, proposing deep cuts to non-defense discretionary spending while increasing funding elsewhere.

These are not abstract numbers. They are warning signs.

What many lawmakers are not willing to acknowledge is that cutting benefits does not eliminate need. It increases suffering.

Families do not suddenly become self-sufficient because a politician decided they should have less help. They do not magically find affordable childcare, stable housing, decent wages, or accessible healthcare. What happens instead is far more devastating. Parents skip meals so their children can eat. Seniors ration medication. Families fall behind on rent. Children carry instability into every part of their lives.

And then the cost shows up anyway. Just not where lawmakers want you to look.

There was a case that should have changed this conversation. In “Million-Dollar Murray,” Malcolm Gladwell reported in The New Yorker that one unhoused man in Reno, Nevada, Murray Barr, cycled through emergency rooms, jails, and crisis services so often that he ended up costing taxpayers nearly $1 million over time.

Not because he received too much help.
Because he received none that actually stabilized his life.

No housing.
No consistent care.
Only crisis.

It would have been far cheaper to house him.

But we do not invest in prevention. We fund emergencies.

And then there is Oklahoma.

In Oklahoma, taxpayers spent roughly $1 million incarcerating a man for decades over a low-level property crime involving stolen electronics. While the man reportedly had a prior criminal history, that detail cannot be separated from the broader conditions that shape so many lives. Where poverty is high, opportunity is scarce, and systems fail to meet basic needs, crime often becomes part of the landscape, not because poverty excuses harm, but because deprivation, instability, and neglect create the conditions in which it thrives.

Let’s pause for a moment and sit with that.

This is the part lawmakers do not say out loud.

Cutting benefits does not save money. It shifts costs into systems that are more expensive, more reactive, and more harmful. Emergency rooms instead of preventative care. Jails instead of housing. Crisis response instead of stability.

The cost is still paid.
Just later.
And in crueler ways.

There is also moral dishonesty in how these cuts are sold. Lawmakers praise hard work while making survival harder for working people. They talk about family values while stripping away the supports families rely on. They speak about responsibility while ignoring that millions of people are already doing everything they can to stay afloat.

What they are really doing is making a choice.

A choice about whose pain matters.
And whose does not.

Because benefits are not luxuries. They are the thin line between stability and crisis for millions of people.

A society should be judged not by how harshly it treats people in need, but by how fiercely it protects them.

When lawmakers cut benefits, they are not promoting independence.

They are manufacturing desperation and creating an economic crisis.

Imagina que te levantas temprano para ir al trabajo y acompañas tu desayuno con un vaso de leche. Días después, desarrollas una tos persistente, flema con sangre, dolor en el pecho y una fatiga que no te deja continuar. Acudes al médico y recibes un diagnóstico de tuberculosis. Sin saberlo, probablemente ya has expuesto a quienes te rodean.

Así comenzó una de las epidemias más devastadoras en Puerto Rico durante el siglo XX. En aquel entonces, la leche no pasteurizada era un vehículo común de transmisión. La tuberculosis llegó a convertirse en la segunda causa de muerte en la isla. Alcanzó su punto más crítico en 1933, con 333 muertes por cada 100,000 personas, sin discriminar entre clases sociales. Ante la falta de tratamiento, se establecieron hospitales en Río Piedras y Ponce, donde los pacientes eran aislados como única estrategia de contención, aunque con capacidad limitada.

Sin embargo, a partir de la década de 1950, la historia cambió drásticamente. La mortalidad se redujo casi a la mitad en pocos años. Esto no fue casualidad. Según el estudio epidemiológico del Dr. Jose Janer en 1954, fue el resultado de intervenciones de salud pública basadas en evidencia: la introducción de antibióticos como la estreptomicina y el ácido para-aminosalicílico (PAS), el acceso gratuito a tratamiento, mejoras en el diagnóstico y campañas educativas efectivas. Puerto Rico demostró que cuando la ciencia y la política pública trabajan de la mano, se salvan vidas.

Hoy, muchos podrían pensar que la tuberculosis es cosa del pasado. Pero la realidad no es así. A nivel global, sigue siendo la enfermedad infecciosa más mortal, sobrepasando a COVID. Cobra más de 1.2 millones de vidas anualmente según la Organización Mundial de la Salud. La tuberculosis es prevenible y curable, pero persiste donde hay pobreza, fallan los sistemas de salud, y la continuidad del tratamiento.

Iniciativas como los programas de la Agencia de los Estados Unidos para el Desarrollo Internacional (USAID) y el Fondo Global de lucha contra el SIDA, la tuberculosis y la malaria han sido pilares en la reducción de millones de casos a nivel global. Estas iniciativas financian el diagnóstico, tratamiento y prevención en países con menos recursos. Sin embargo, cambios recientes en financiamiento y prioridades políticas amenazan con debilitar estos avances. Reducir el apoyo a estos esfuerzos aumenta el riesgo de que la enfermedad resurja y cruce fronteras.

Puerto Rico no es la excepción. Este 24 de marzo, Día Mundial de la Tuberculosis, y como defensora comunitaria del altruismo global con la organización nacional RESULTS, les exhorto a actuar. Informarnos, hablar del tema, apoyar organizaciones que trabajan en salud global y exigir a nuestros líderes que mantengan el financiamiento a programas que han probado funcionar. La erradicación de la tuberculosis no depende solo de avances científicos, sino de decisiones colectivas. Olvidar nuestra historia no la hace desaparecer, pero nos hace más vulnerables a repetirla.
